摘 要:目的:探讨膝关节站立负重位数字X线摄影(DR)的应用效果。方法:随机选择2022年9月—2023年8月在山东省立医院菏泽医院进行X线检查的400例疑似膝关节病变患者,分别采用膝关节站立负重位、常规膝关节正侧位DR进行检查,各200例,以临床医生综合判断结果为评判标准,比较两组诊断膝关节病变的结果、不同类型病变检出准确率、关节间隙变窄程度及膝关节关节面夹角。结果:膝关节站立负重位DR诊断膝关节病变的一致性良好,Kappa值=0.855;正侧位DR一致性尚可,Kappa值=0.406。负重位DR诊断不同病变类型的准确率均高于正侧位,差异有统计学意义(P<0.05);负重位DR对膝关节间隙狭窄的显示更加明显,膝关节关节面夹角大于正侧位,差异有统计学意义(P<0.05)。结论:与常规膝关节正侧位DR诊断比较,膝关节站立负重位DR对膝关节损伤的诊断价值更高,在临床可推广应用。Objective To explore the application effect of digital radiography(DR)in knee joint standing weight-bearing position.Methods 400 Patients with suspected knee joint lesions were randomly selected for X-ray examination in Heze Hospital,Shandong Provincial Hospital from September 2022 to August 2023,and 200 cases were respectively examined by standing knee weight-bearing position and conventional knee joint anterior and lateral DR,and the comprehensive judgment results of clinicians were used as the evaluation criteria.The results of diagnosis of knee joint lesions,detection accuracy of different types of lesions,narrowing degree of joint space and Angle of joint surface of knee joint were compared between the two groups.Results The consistency of DR in the diagnosis of knee joint disease was very good,Kappa value=0.855;The consistency of anteroposterior and lateral DR was general,Kappa value=0.406.The accuracy of DR in diagnosis of different lesion types in weight-bearing position was higher than that in anterior-lateral position(P<0.05).DR in weight-bearing position showed more obvious knee space stenosis,and the Angle of joint surface of knee joint was larger than that in anterolateral position(P<0.05).Conclusion Compared with the conventional anterolateral DR of knee joint,DR of knee joint standing weight-bearing position has higher diagnostic value for knee joint injury,and can be popularized in clinical application.
