咪喹莫特乳膏联合光动力疗法治疗外阴尖锐湿疣患者的效果

摘　　要：目的:观察咪喹莫特乳膏联合光动力疗法治疗外阴尖锐湿疣患者的效果。方法:选取2021年8月至2023年8月该院收治的98例外阴尖锐湿疣患者进行前瞻性研究,按照随机数字表法将其分为对照组和研究组各49例。对照组采用光动力疗法治疗,研究组在对照组基础上联合咪喹莫特乳膏治疗,比较两组临床疗效、治疗前后炎性指标[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)]水平、血清再生因子指标[血管内皮生长因子(VEGF)、缺氧诱导因子-1α(HIF-1α)]水平、不良反应发生率和复发率。结果:研究组治疗总有效率为95.92%(47/49),高于对照组的83.67%(41/49),差异有统计学意义(P<0.05);治疗后,两组TNF-α、CRP、IL-6、VEGF、HIF-1α水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);随访3个月,研究组复发率为0,低于对照组的12.20%(5/41),差异有统计学意义(P<0.05)。结论:咪喹莫特乳膏联合光动力疗法治疗外阴尖锐湿疣患者可提高治疗总有效率,降低炎性指标水平、血清再生因子指标水平和复发率,效果优于单纯光动力疗法治疗。Objective:To observe effects of Imiquimod cream combined with photodynamic therapy in treatment of patients with condyloma acuminatum of vulva.Methods:A prospective study was conducted on 98 patients with condyloma acuminatum of vulva admitted to the hospital from August 2021 to August 2023.According to the random number table method,they were divided into control group and study group,49 cases in each group.The control group was treated with photodynamic therapy,while the study group was treated with Imiquimod cream on the basis of that of the control group.The clinical efficacy,the levels of inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP)]and serum regeneration factor indexes[vascular endothelial growth factor(VEGF),hypoxia-inducible factor-1α(HIF-1α)],the incidence of adverse reactions,and the recurrence rate were compared between the two groups before and after the treatment.Results:The total effective rate of the study group was 95.92%(47/49),which was higher than 83.67%(41/49)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of TNF-α,CRP,IL-6,VEGF and HIF-1αin the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 3 months of follow-up,the recurrence rate of the study group was 0,which was lower than 12.20%(5/41)of the control group,and the difference was statistically significant(P<0.05).Conclusions:Imiquimod cream combined with photodynamic therapy in the treatment of the patients with condyloma acuminatum of vulva can improve the total effective rate of treatment and reduce the levels of inflammatory factors and serum regeneration factor indexes and the recurrence rate.Moreover,it is superior to single photodynamic therapy.
