血府逐瘀胶囊联合低分子肝素钙注射液在人工髋关节置换术后抗凝中的应用效果

摘　　要：目的:观察血府逐瘀胶囊联合低分子肝素钙注射液在人工髋关节置换术后抗凝中的应用效果。方法:选取2021年5月至2023年5月该院收治的126例人工髋关节置换术后患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各63例。对照组采用低分子肝素钙注射液抗凝,观察组在对照组基础上联合血府逐瘀胶囊抗凝,比较两组下肢周径差、视觉模拟评分法(VAS)评分、凝血-纤溶指标[活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、D-二聚体(D-D)、纤维蛋白原(FIB)、纤维蛋白降解产物(FDP)]水平、下肢深静脉血栓(DVT)发生率和不良反应发生率。结果:治疗2周后,两组大腿、小腿周径差低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05);治疗1、2周后,两组VAS评分低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05);治疗2周后,两组APTT、PT长于治疗前,且观察组长于对照组,两组D-D、FIB、FDP水平低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05);观察组下肢DVT发生率为0,低于对照组的9.52%,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:血府逐瘀胶囊联合低分子肝素钙注射液应用于人工髋关节置换术后患者可降低下肢周径差、VAS评分和下肢DVT发生率,改善凝血-纤溶指标水平,效果优于单纯低分子肝素钙注射液抗凝。Objective:To observe effects of Xuefu Zhuyu capsules combined with low molecular weight heparin calcium injection on anticoagulation after total hip arthroplasty.Methods:A prospective study was conducted on 126 patients after total hip arthroplasty admitted to the hospital from May 2021 to May 2023.According to the random number table method,they were divided into observation group and control group,63 cases in each group.The control group was treated with low molecular weight heparin calcium injection for anticoagulation,while the observation group was combined with Xuefu Zhuyu capsules for anticoagulation on the basis of that of the control group.The lower extremity circumference difference,the pain visual analogue scale(VAS)score,the coagulation-fibrinolysis indexes[activated partial thromboplastin time(APTT),prothrombin time(PT),D-dimer(D-D),fibrinogen(FIB),fibrin degradation product(FDP)]levels,the incidence of lower limb DVT,and the incidence of adverse reactions were compared between the two groups.Results:After 2 weeks of treatment,the circumference differences of thigh and calf in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 1 and 2 weeks of treatment,the VAS scores of the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 2 weeks of treatment,the levels of APTT and PT in the two groups were longer than those before the treatment,and those in the observation group were longer than those in the control group;the levels of D-D,FIB and FDP in the two groups were lower than those before the treatment,and those in the observation group were lower than those in the control group;and the differences were statistically significant(P<0.05).The incidence of lower extremity DVT in the observation group was 0,which was lower th
