数字X线摄影和CT影像检查在脊柱爆裂性骨折中的诊断价值分析

摘　　要：目的:分析数字X线摄影(DR)与CT影像检查在脊柱爆裂性骨折中的诊断价值。方法:选取2020年1月—2023年7月黔西南州中医医院收治的疑似脊柱爆裂性骨折患者50例作为研究对象。所有患者接受DR和CT影像检查,以CT造影结果作为诊断“金标准”,分析两种检查方式的诊断效能、检查时间、检查费用、对不同疾病症状的检出率。结果:CT造影结果显示,50例疑似脊柱爆裂性骨折患者中阳性39例,阴性11例。CT影像检查的灵敏度、特异度、准确率均高于DR检查,差异有统计学意义(P<0.05);CT影像检查的检查时间与检查费用高于DR检查,差异有统计学意义(P<0.001);CT影像检查对横突或棘突骨折、椎体直径宽、椎弓根间距加宽、椎管狭窄、小关节脱位的检出率高于DR检查,差异有统计学意义(P<0.05)。结论:CT影像检查在脊柱爆裂性骨折中的诊断效能较高,但是成本也较高,而DR检查成本较低,临床可根据患者实际情况合理选择检查方式。Objective:To analyze the diagnostic value of digital radiography(DR)and CT imaging examination in spinal burst fracture.Methods:Fifty patients with suspected spinal burst fractures admitted to Qianxinan Prefecture Hospital of Traditional Chinese Medicine from January 2020 to July 2023 were selected as the study subjects.All the patients were examined by DR and CT imaging,and the CT imaging results were used as the"gold standard"for diagnosis to analyze the diagnostic efficacy of the two modes of examination,the examination time,the examination cost,and the detection rate of different disease symptoms.Results:The results of the"CT imaging"showed that thirty-nine of the fifty patients with suspected spinal burst fractures were positive and eleven were negative.The sensitivity,specificity,and accuracy of CT imaging were higher than those of DR imaging,and the difference was statistically significant(P<0.05);the examination time and examination cost of CT imaging examination were higher than that of DR examination,and the difference was statistically significant(P<0.001);the detection rate of transverse or spinous process fracture,vertebral body diameter widening,pedicle spacing widening,spinal canal stenosis,and subluxation of small joints by CT imaging examination was higher than that of DR examination,and the difference was statistically significant(P<0.05).Conclusion:CT imaging examination has higher diagnostic efficacy in spinal burst fracture,but it also has higher cost,while DR examination has lower cost,and the clinic can reasonably choose the examination mode according to the actual situation of patients.
