磁共振mapping技术定量在2型糖尿病患者早期肾功能评估中的应用

摘　　要：目的 探讨磁共振mapping技术定量在2型糖尿病(T2DM)患者早期肾功能评估中的应用价值。方法 本文为病例对照研究。按照1:1配对原则选取2021年5月至2022年7月诊治的18例轻度DN患者(eGFR> 60mL/min·1.73m^(2))、18例中度DN患者(eGFR 60~30 mL/min·1.73m^(2))、18例重度DN患者(eGFR<30mL/min·1.73m^(2)),共计54例DN患者作为研究对象,设立为DN组,同期按照选取54例单纯2型糖尿病患者设立为对照组。对比T_(1)值、T_(2)值,采用ROC曲线模型诊断DN和预测DN患者肾功能的曲线下面积(AUC)、敏感度、特异度。结果 DN组的T_(1)值、T_(2)值高于对照组(P<0.05)。重度组的T_(1)值、T_(2)值均高于轻度组和中度组(P<0.05)。ROC曲线分析显示,T_(1)值、T_(2)值及综合参数诊断DN的AUC值分别为(0.801.0.809、0.840,P<0.05),敏感度分别为66.70%、63.00%、 72.20%;特异度分别为85.20%、90.70%、88.90%。T_(1)值、T_(2)值及综合参数预测DN患者肾功能的AUC值分别为(0.782、0.770、0.824,P<0.05);敏感度分别为77.80%、72.20%;特异度分别为72.20%、97.20%、86.10%。结论 磁共振mapping技术可定量评估2型糖尿病患者的肾功能,有助于早期发现DN。Objective To investigate the quantitative application value of magnetic resonance mapping technique in early renal function assessment of patients with type 2 diabetes mellitus(T2DM).Methods This is a case-control study.Eighteen patients with mild DN(eGFR> 60mL/min·1.73m^(2))and 18 patients with moderate DN(eGFR 60-30) were selected from May 2021 to July 2022according to the 1:1 pairing principle mL/min·1.73m^(2),18 patients with severe DN(eGFR <30mL/min·1.73m^(2)),a total of 54 patients with DN were selected as the study object,and 54patients with simple type 2 diabetes were selected as the control group during the same period.Comparing T_(1) and T_(2) values,the ROC curve model was used to diagnose DN and predict the AUC,sensitivity and specificity of renal function in patients with DN.Results The values of T_(1) and T_(2) in DN group were higher than those in T2DM group(P<0.05).The values of T_(1) and T_(2) in severe group were higher than those in mild and moderate groups(P<0.05).ROC curve analysis showed that the AUC values of T_(1),T_(2) and comprehensive parameters were(0.801,0.809,0.840,P<0.05),and the sensitivity were 66.70%,63.00%,72.20%,The specificity was 85.20%,90.70%and 88.90%.T_(1),T_(2) and combined parameters predicted the AUC values of renal function in DN patients(0.782,0.770,0.824,P<0.05),The sensitivity,respectively.Conclusion Magnetic resonance mapping can quantitatively evaluate renal function in patients with type 2 diabetes,which is helpful for early detection of DN.
