超声引导下腰方肌阻滞联合纳布啡对老年THA患者的影响

摘　　要：目的:探究采用超声引导下腰方肌阻滞联合纳布啡应用于老年全髋关节置换术(THA)患者的效果。方法:选取医院收治的老年THA患者为研究对象,随机分为两组,各50例,试验组术毕于患侧在超声引导下行腰方肌阻滞,对照组不进行此操作,比较两组镇静镇痛情况、髋关节活动情况、认知功能。结果:试验组术后Ramsay、视觉模拟评分法(VAS)评分均低于对照组(P<0.05);试验组术后48h简易精神状态检查量表(MMSE)评分,术后12h、24h、48h髋关节最大屈曲度与外展活动度高于对照组(P<0.05)。结论:超声引导下腰方肌阻滞联合纳布啡可降低老年THA患者Ramsay与VAS评分,改善认知功能与髋关节活动情况。Objective:To investigate the effect of ultrasound-guided quadratus lumborum block combined with nalbuphine on elderly patients undergoing THA.Methods:Elderly patients who underwent THA were selected and randomly divided into two groups.After surgery,the treatment group received ultrasound-guided quadratus lumborum block,while the control group did not receive such block.The two groups were given multimodal analgesia with intravenous injection of nalbuphine(5mg)every 24 hours and nerve block.The situation of sedation and analgesia,motion of hip joint,and cognitive function were compared between groups.Results:Ramsay scores and VAS scores of the treatment group after surgery were lower than those of the control group(P<0.05).MMSE scoreof the treatment group at 48 h after surgery,and maximum range of flexion and range of abduction of hip joint were higher/greater compared to the control group(P<0.05).Conclusion:Ultrasound-guided quadratus lumborum block combined with nalbuphine can reduce Ramsay score and VAS score of elderly patients undergoing THA,improve their cognitive function and motion of hip joint.
