超声造影定量参数联合血清学指标评估克罗恩病炎症活动度的临床价值

摘　　要：目的探讨超声造影定量参数联合血清学指标评估克罗恩病(CD)炎症活动度的临床价值。方法选取于我院就诊的CD患者95例,依据是否处于炎症活动期分为炎症活动期组67例和非炎症活动期组28例,比较两组超声造影定量参数[达峰时间(TTP)、上升时间(RT)、峰值强度(PI)]和血清学指标[血清微小RNA-223(miR-223)、白细胞介素6(IL-6)]的差异。绘制受试者工作特征(ROC)曲线分析超声造影定量参数、血清学指标单独及联合应用对CD炎症活动度的评估价值。结果与非炎症活动期组比较,炎症活动期组TTP、RT均减低,PI及血清miR-223、IL-6均增高,差异均有统计学意义(均P<0.05)。ROC曲线分析显示,TTP、RT、PI、血清miR-223、IL-6单独及联合应用评估CD炎症活动度的曲线下面积分别为0.735、0.703、0.796、0.858、0.805、0.962,灵敏度、特异度分别为100%、97.0%、80.6%、73.1%、50.7%、97.0%和50.0%、46.4%、67.9%、89.3%、100%、96.4%。结论超声造影定量参数联合血清学指标评估CD炎症活动度具有较高的临床价值。Objective To explore the clinical value of contrast-enhanced ultrasound quantitative parameters combined with serum indexes in the evaluation of inflammatory activity in Crohn’s disease(CD).Method A total of 95 patients with CD who visited our hospital were selected and divided into inflammatory active phase group(67 cases)and non inflammatory active phase group(28 cases)according to whether they were in the inflammatory active phase.The differences of contrast-enhanced ultrasound quantitative parameters[time to peak(TTP),rise time(RT),peak intensity(PI)]and serum indexes(miR-223,IL-6)were compared between the two groups.Receiver operating characteristic(ROC)curve was drawn to analyze the value of contrast-enhanced ultrasound quantitative parameters,serum indexes applied alone and their combination in evaluating the inflammatory activity in CD.Results Compared with the non inflammatory active phase group,the inflammatory active group showed a decreasing in TTP and RT,and an increasing in PI and serum miR-223,IL-6,with statistical significance(all P<0.05).ROC curve analysis showed that the area under the curve of TTP,RT,PI,serum miR-223,IL-6,and their combined application for evaluating inflammation activity in CD were 0.735,0.703,0.796,0.858,0.805,and 0.962.The sensitivity and specificity were 100.0%,97.0%,80.6%,73.1%,50.7%,97.0%and 50.0%,46.4%,67.9%,89.3%,100%,96.4%,respectively.Conclusion The combination of contrast-enhanced ultrasound quantitative parameters and serum indexes has high evaluation value for CD inflammation activity.
