摘　　要：报告1例替雷利珠单抗(PD-1抑制剂)靶向治疗后引起皮肤不良反应。患者男,63岁。使用PD-1抑制剂后躯干和四肢出现红斑、斑丘疹及斑块半年,加重1个月。皮肤科检查:双下肢见泛发性暗紫红色斑块和斑片,表面呈蛎壳样外观,Auspitiz征(+);躯干部及双上肢散在分布红斑、丘疹及斑丘疹,部分中央溃疡及结痂;双侧手背见数片紫红斑块,表面见白色网状纹;右手掌心、左手大鱼际见胼胝样黄色斑块,呈衣领样外观。诊断:替雷利珠单抗致皮肤不良反应。治疗:PD-1继续治疗,对症处理后皮损明显消退。A case of psoriatic and lichen planus rash after targeted therapy with Tirelizumab(PD-1 inhibitor)is reported.The patient was a 63-year-old male.After using PD-1 inhibitor for half a year,psoriasis-like rash and lichen planus-like rash appeared on her trunk and limbs,and aggravated in the past month.Dermatological examination showed generalized dark purplish plaques and patches on both lower limbs with an oystercatch-like appearance.Auspitiz sign was positive.There were scattered erythema,papules and maculopapules on the trunk and upper limbs,some of which were cracked and crusted in the center.Several pieces of purplish erythematous patches with white reticular lines were seen on the dorsum of both hands.A callous yellow plaque was seen in the palm of the right hand and the thenar of the left hand,showing a collar-like appearance.A diagnosis of skin adverse reaction of PD-1 inhibitor was proposed.After continuous treatment with PD-1 therapy,the rash disappeared significantly.
