摘　　要：目的分析静脉用药医嘱审核中存在的假阳性和假阴性问题,完善静脉用药医嘱前置审核系统的规则设置,促进静脉用药合理使用。方法调取2022年2月20日~3月20日住院患者静脉用药医嘱的所有不合理问题,进行回顾性分析。通过查询循证证据,必要时与临床沟通反馈,优化静脉用药审核规则库。结果静脉用药审核规则优化后,静脉用药调配中心不适宜医嘱占比降低了83.01%,差异有统计学意义(P<0.001),同时药师干预问题处方成功率明显提高。结论基于循证证据结合临床需求构建个性化的审核规则库,有助于提高处方审核质量和静脉用药合理化水平,保障患者静脉用药安全。Objective To analyze the false-positive and false-negative problems existing in the examination of intravenous administration prescriptions,improve the rules of the pre-review system of intravenous administration orders,and promote the rational use of intravenous administration.Methods All unreasonable problems of intravenous administration orders of inpatients from February 20 to March 20,2022 were collected and analyzed retrospectively.By inquiring evidence-based evidence and contacting with clinicians when necessary,to optimize the review rules of intravenous administration.Results After optimizing the prescription review rules for intravenous administration,the proportion of inappropriate prescriptions in the pharmacy intravenous admixture services(PIVAS)decreased by 83.01%,and the difference was statistically significant(P<0.001).Additionally,the success rate of pharmacists’intervention in inappropriate prescriptions was significantly improved.Conclusion Based on evidence-based evidence and clinical needs,the establishment of personalized audit rules is helpful to improve the quality of prescription review and the rationalization level of intravenous administration,and to ensure the safety of intravenous administration for patients.
