2022~2023年某院清热类中药饮片的使用情况及合理性分析

摘　　要：目的探讨2022~2023年清热类中药饮片的使用情况及合理性分析,为采购和临床合理用药提供参考。方法采用Excel软件对2022年1月~2023年12月清热类中药饮片的用量情况和销售金额情况进行统计分析,并抽取268张用药处方,比较和分析导致患者发生的不良反应症状、累及系统情况和产生的原因。结果在清热类中药饮片用量情况排名中,前十名的品种比较稳定,其中蒲公英和白花蛇舌草稳居前2位。在清热类中药饮片销售金额情况排名中,前十名的品种相对稳定,但每年的销售金额情况排名都会有略微变动。在清热类中药饮片268份处方药中,不良反应处方药有124张,主要累及皮肤黏膜系统,症状是皮疹,发生率为20.52%;其次累及消化系统和其他系统,症状分别是恶心呕吐和肢体厥冷、血压下降,发生率分别为4.85%和14.93%;还累及生殖系统和呼吸系统,症状分别为阴道出血和呼吸急促,发生率分别为2.61%和3.36%。不良反应发生原因包括配伍不当、剂量不当、药物因素、辨证不准、个体因素和其他。结论在2022~2023年清热类中药饮片的品种比较稳定,但个别饮片的用量情况和销售金额情况的变化显著,而且清热类中药饮片的处方有部分出现不良反应症状,因此清热类中药饮片在使用的过程中应该仔细辨证和加强管理,做好用药指导,加强合理用药宣传,对中药饮片的质量做好把控,确保患者合理和安全使用清热类中药饮片。Objective To investigate the use and rationality of heat-clearing herbal decoction pieces in my hospital from 2022 to 2023,and to provide reference for rational drug use in the clinic.Methods Excel software was used to analyze the consumption and consumption amount of heat-clearing TCM decoction pieces from January 2022 to December 2023,and 268 prescriptions were selected to compare and analyze the symptoms,involved systems and causes of adverse reactions in patients.Results In the ranking of the dosage of heat-clearing Chinese medicine decoction pieces,the top 10 varieties are relatively stable,among which dandelion and white-flowered snake tongue grass are firmly in the top 2.In the ranking of the sales amount of heat-clearing traditional Chinese medicine decoction pieces,the top ten varieties are relatively stable,but the ranking of the sales amount will change slightly every year.Among the 268 prescription drugs of heat-clearing Chinese medicine decoction pieces,there were 124 prescription drugs with adverse reactions,mainly involving the skin and mucosal system,with the incidence of rash(20.52%),followed by the digestive system and other systems,with the incidence of nausea and vomiting,cold extremities and decreased blood pressure,with the incidence of 4.85% and 14.93%,respectively,and the reproductive system and respiratory system,with vaginal bleeding and shortness of breath,with the incidence of 2.61% and 3.36%,respectively.The causes of adverse reactions included improper compatibility,improper dosage,drug factors,dialectical inaccuracy,individual factors and others.Conclusion In 2022~2023,the varieties of heat-clearing Chinese medicine decoction pieces are relatively stable,but the dosage and sales amount of individual decoction pieces have changed significantly,and some of the prescriptions of heat-clearing Chinese medicine decoction pieces have adverse reaction symptoms,so in the process of use,the heat-clearing Chinese medicine decoction pieces should be carefully differentiated and management,medic
