胶体金免疫层析法检测皮肤癣菌的临床适用性评估

摘　　要：目的评估与直接镜检相比,胶体金免疫层析法诊断皮肤癣菌感染的临床适用性。方法收集346例由门诊医生怀疑皮肤癣菌感染的皮屑或甲屑标本,将其分为3份,分别行直接镜检和免疫层析抗原检测,两种方法结果不一致者行PCR。比较两种方法诊断结果的一致率和不一致率。结果抗原检测和直接镜检的阳性检出率分别为76.59%、69.36%。346例临床样本中,293例抗原检测与直接镜检结果一致,一致率为84.7%;53例两种方法检测结果不一致,不一致率为15.3%。不一致的临床样本中46份行PCR进一步鉴定,分别有39例和7例与抗原检测和直接镜检结果相同。即皮肤癣菌抗原检测与PCR校正后的直接镜检结果一致(阳性或阴性)的标本数为332份,一致率为96.0%,不一致率为4.0%。结论免疫层析法是简单、可靠的真菌检测方法,灵敏度比直接镜检高,有着很好的临床实用价值。Objective To evaluate the clinical applicability of colloidal gold immunochromatography in the diagnosis of dermatomycosis infection compared with fungal microscopic examination.Methods A total of 346 samples of skin or nail-scraps suspected of superficial fungal infection were collected from the Dermatology Hospital of Chinese Academy of Medical Sciences.The samples were divided into 3 parts,and fungal microscopic examination and immunochromatography antigen detection were performed,respectively.PCR was performed in cases when the results of the two methods were inconsistent.The consistency rate and inconsistency rate of the two methods were compared.Results The positive rate of antigen detection and fungal microscopic examination is 76.59%and 69.36%,respectively.Among 346 clinical samples,293 cases are consistent between two methods,and the consistency rate was 84.7%.The results of 53 cases were inconsistent between the two methods,and the inconsistency rate was 15.3%.46 of the incongruent clinical samples were further identified by PCR,of which 39 cases and 7 cases were identical with antigen detection and direct microscopic examination,respectively.That is,332 samples were consistent with the direct microscopic examination results(positive or negative)after PCR correction,with a consistent rate of 96.0%and an inconsistent rate of 4.0%.Conclusion Immunochromatography is a simple and reliable method for fungal detection.It is more sensitive than fungal microscopic examination and has good clinical value.
