摘  要:目的 探讨多模态超声经阴道二维、三维超声及静脉超声造影(CEUS)对子宫肌壁间妊娠(IMP)的诊断价值。方法 回顾性分析收治的3例IMP的超声表现及临床资料。结果 2例二维超声误诊为宫角或间质部妊娠,1例漏诊;1例三维超声误诊为宫角或间质部妊娠,1例诊断为IMP,1例结合宫腔镜考虑IMP;2例CEUS诊断为IMP。2例行宫腹腔镜手术,1例行保守治疗。结论 IMP是一种罕见的异位妊娠,二维超声易误诊,三维超声可直观显示病灶与宫腔的关系,如果两者分界欠清晰或不清晰,CEUS可明确病灶与肌层及宫腔的关系。Objective To investigate the diagnostic value of multimodal ultrasound transvaginal two-dimension-al,three-dimensional ultrasound,and contrast-enhanced ultrasound(CEUS)in intramural ectopic pregnancy(IMP).Methods A retrospective analysis was performed on the ultrasonographic features and clinical data of 3 cases of IMP.Results Two cases were misdiagnosed as horn or interstitial pregnancy and 1 was missed by two-dimensional ultra-sound.With three-dimensional ultrasound,one case was misdiagnosed as horn or interstitial pregnancy and one was diagnosed as IMP,and the third case was diagnosed as IMP combined with the result of hysteroscopy.Two cases were diagnosed as IMP by CEUS.Two cases were treated by hysteroscopy and laparoscopy.and one by conservative treat-ment.Conclusions IMP is a rare ectopic pregnancy,which can be easily misdiagnosed by two-dimensional ultra-sound.The relationship between the lesion and uterine cavity might be realized directly by three-dimensional ultra-sound.CEUS can make clear relationship between them when the distinction is unclear.
