摘　　要：目的 观察肩袖损伤采用肌骨超声引导下注射高渗葡萄糖与曲安奈德治疗的疗效对比研究。方法 选取2021年3月至2022年11月郑州市骨科医院收治行肌骨超声引导下注射治疗的肩袖损伤患者。按随机数字表法分为高糖组和激素组,肌骨超声引导下高糖组注射利多卡因及高渗葡萄糖,激素组注射利多卡因及曲安奈德。比较两组治疗前后肩关节评分系统(University of California at LosAngeles scoring system, UCLA scoring system)评分、疼痛视觉模拟评分(visual analogue scale, VAS)、肩关节主动前屈关节活动度(AFROM)和主动外展关节活动度(AAROM)。结果 激素组和高糖组治疗前VAS评分比较无统计学意义(P>0.05),治疗后的VAS评分均较治疗前VAS评分下降(P均<0.001)。两组组间比较,激素组治疗1、3次后、治疗3个月后VAS评分分别为(3.4±0.2)分、(2.1±0.1)分及(1.6±0.2)分,均低于高糖组(P均<0.05);激素组和高糖组治疗前UCLA评分比较无统计学意义(P>0.05),治疗后的UCLA评分均较治疗前UCLA评分下降(P均<0.001)。两组组间比较,激素组治疗1、3次后、治疗3个月后UCLA评分分别为(22.9±1.8)分、(24.5±2.1)分及(29.3±1.5)分,均高于高糖组(P均<0.05);治疗前两组AFROM、AAROM角度变化比较无统计学意义(P>0.05),治疗后均提升(P均<0.05),且激素组AFROM、AAROM角度变化评分高于高糖组(P均<0.05)。结论 肩袖损伤采用肌骨超声引导下注射高渗葡萄糖与曲安奈德注射液均效果显著,且注射3个月内注射曲安奈德疗效好于注射高渗葡萄糖,但对激素类药物使用比较谨慎的部分患者高渗葡萄糖注射治疗是另一个有效的选择。Objective To observe the effect of musculoskeletal ultrasound-guided injection of hypertonic glucose and triamcinolone olone in the treatment of rotator cuff injury.Methods Patients with rotator cuff injury who received musculoskeletal ultrasound guided injection treatment in Zhengzhou Orthopaedic Hospital from March 2021 to November 2022 were selected. They were divided into hyperglycemic group and hormone group according to random number table method. Hyperglycemic group was injected with lidocaine and hypertonic glucose under musculoskeletal ultrasound guidance, while hormone group was injected with lidocaine and triamcinolone olone. The shoulder joint scoring system(University of California at LosAngeles scoring system, University of California at Losangeles Scoring System, UCLA scoring system), visual analogue scale(VAS), active anterior flexion range of motion(AFROM) and active abduction range of motion(AAROM).Results There was no significant difference in VAS scores between hormone group and hyperglycemic group before treatment(P>0.05), and VAS scores after treatment were lower than those before treatment(P<0.001). Compared between the two groups, the VAS scores of the hormone group were(3.4±0.2),(2.1±0.1) and(1.6±0.2) points after one treatment, three courses of treatment and 3 months after treatment, respectively, which were lower than those of the hyperglycemic group(P<0.05). There was no statistical significance in UCLA scores between the hormone group and the high glucose group before treatment(P>0.05), and the UCLA scores after treatment were lower than those before treatment(P<0.001). Compared between the two groups, the UCLA scores in the hormone group were(22.9±1.8),(24.5±2.1) and(29.3±1.5) points after one treatment, three courses of treatment and 3 months after treatment, respectively, which were higher than those in the hyperglycemic group(P<0.05).
