甲状腺肿瘤累及气管手术后气管造瘘延期修补的临床观察

摘　　要：目的探讨在基层医院针对甲状腺肿瘤累及气管行手术治疗遗留的气管瘘,进行延期修补的方法和效果。方法针对三个医院的六例因为甲状腺肿瘤累及气管壁,行甲状腺肿瘤以及累及的气管壁切除后,行气管造瘘,3-6月后根据情况行一次或多次气管瘘修补。结果术后经过2-6年的随访,4例患者呼吸良好,声音略有嘶哑。2例患者活动后略有喉鸣,不影响生活,均属于60岁以上的老年病例。未见肿瘤复发。结论针对甲状腺肿瘤累及气管的病例,可以根据情况先行气管造瘘,后延期行气管瘘修补手术,值得推广使用。Objective To investigate the method and effect of delayed repair of trachea fistula left after surgical treatment of thyroid tumor involving trachea in primary hospitals.Methods Six patients in three hospitals were involved in trachea wall due to thyroid tumor.After the removal of thyroid tumor and involved trachea wall,tracheostomy was performed.After 3-6 months,trachea fistula repair was performed once or more according to the situation.Results After 2-6 years of follow-up,4 patients had good breath and slight hoarseness.Two patients had slight voice after activity,which did not affect their life.They were all elderly patients over 60 years old.No tumor recurrence was found.Conclusion For the cases of thyroid tumor involving trachea,tracheostomy can be performed first according to the situation,and then the repair of trachea fistula can be delayed,which is worth popularizing.
