摘  要:目的探讨鼻腔黏膜糜烂引起鼻出血的临床治疗方法,观察治疗后黏膜恢复情况,探索最佳的精细化治疗方案。方法选取137例门诊就诊的鼻腔黏膜糜烂引起鼻出血的患者进行不同方式治疗,出血明显的使用双极电凝止血后用涂有牛碱性成纤维细胞生长因子凝胶(贝复新批准文号:国药准字S2004001)的明胶海绵贴敷,局部渗血的使用纱泰祺可吸收止血纱贴敷;3d后门诊换药后使用生理性海水冲洗鼻腔,然后局部涂抹贝复新凝胶,随访8~12w,鼻内窥镜记录黏膜恢复情况,对治疗前后进行患者主观感受和客观变化统计学分析。结果137例患者治疗前、后主观症状及内镜下黏膜客观变化有统计学意义(P<0.01)。结论鼻腔黏膜糜烂引起的鼻出血可选择精细化治疗方案,可以有效地控制出血及修复鼻腔黏膜。Objective To explore the clinical treatment of epistaxis caused by nasal mucosal erosion,observe the mucosal recovery after treatment,and explore the best refined treatment.Methods A total of 137 outpatients with epistaxis caused by nasal mucosal erosion were treated in different ways.Effect of Bipolar Electrocoagulation on Bleeding,followed by gelatin sponge coated with bovine alkaline growth factor gel.Sataiqi absorbable hemostatic gauze was used for local bleeding.After 3 days of dressing change,physiological seawater was used to rinse the nasal cavity,and then bovine alkaline growth factor gel was smeared locally.The mucosal recovery was recorded by nasal endoscope and the subjective feelings and objective statistical analysis were performed before and after treatment follow-up was 8~12w.Results The subjective symptoms and endoscopic mucosal changes of 137 patients before and after treatment were statistically significant(P<0.01).Conclusion Refined treatment can be selected for epistaxis caused by nasal mucosal erosion,which can effectively control bleeding and repair nasal mucosa.
