摘　　要：目的:分析微创经皮空心螺钉内固定治疗骨盆前环骨折的临床效果。方法:选取2020年3月—2022年3月北京市上地医院和北京大望路急诊抢救医院收治的骨盆前环骨折患者66例作为研究对象,根据随机数字表法分为对照组与观察组,各33例。对照组实施开放复位内固定术,观察组实施微创经皮空心螺钉内固定术。比较两组治疗效果。结果:观察组骨折复位总优良率高于对照组,差异有统计学意义(P=0.039)。观察组术中出血量少于对照组,术后下床活动时间、骨痂形成时间早于对照组,差异有统计学意义(P<0.001)。术后3、6、8个月,两组骨盆功能评分逐渐升高,且观察组高于对照组,差异有统计学意义(P<0.001)。观察组并发症总发生率低于对照组,差异有统计学意义(P=0.046)。结论:微创经皮空心螺钉内固定治疗骨盆前环骨折的效果显著,可减少术中出血量,加速康复,提高骨折复位优良率,降低并发症发生率。Objective:To analyze the clinical effect of minimally invasive percutaneous hollow screw internal fixation in the treatment of anterior pelvic ring fracture.Methods:A total of 66 patients with anterior pelvic ring fracture admitted to Beijing Shangdi Hospital and Beijing Dawang Road Emergency Rescue Hospital from March 2020 to March 2022 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 33 cases in each group.The control group received open reduction internal fixation and the observation group received minimally invasive percutaneous hollow screw internal fixation.The treatment effects in two groups were compared.Results:The overall excellent rate of fracture reduction in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.039).The intraoperative blood loss in the observation group was less than that in the control group,the postoperative ambulation time and callus formation time in the observation group were earlier than those in the control group,and the difference was statistically significant(P<0.001).At 3,6 and 8 months after surgery,the pelvic function scores in the two groups gradually increased,the scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.001).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.046).Conclusion:Minimally invasive percutaneous hollow screw internal fixation has a significant effect in the treatment of anterior pelvic ring fracture,and can reduce intraoperative blood loss,accelerate recovery,increase the excellent rate of fracture reduction,and reduce the incidence of complications.
