经环甲间隙裸化甲状腺上极血管入路在甲状腺结节切除术中的应用效果分析

摘　　要：目的:分析经环甲间隙裸化甲状腺上极血管入路在甲状腺结节切除术中的应用效果。方法:选取2021年6月—2022年6月于滨州市中心医院行甲状腺切除手术的80例患者作为研究对象,根据随机数字表法分为对照组和试验组,各40例。对照组实施胸骨舌骨肌-胸骨甲状肌间隙入路,试验组实施经环甲间隙裸化甲状腺上极血管入路。比较两组治疗满意度、并发症发生情况、术中喉上神经寻找时间、生活质量。结果:试验组治疗总满意度高于对照组,差异有统计学意义(P=0.013)。试验组并发症总发生率低于对照组,差异有统计学意义(P=0.043)。试验组术中喉上神经寻找时间短于对照组,差异有统计学意义(P=0.036)。术前,两组社会功能、情绪职能、躯体疼痛、生理功能、总体健康评分比较,差异无统计学意义(P>0.05);术后,两组社会功能、情绪职能、躯体疼痛、生理功能、总体健康评分高于术前,且试验组高于对照组,差异有统计学意义(P<0.05)。结论:经环甲间隙裸化甲状腺上极血管入路在甲状腺结节切除术中的应用效果显著,可缩短术中喉上神经寻找时间,降低并发症发生率,提高患者治疗满意度和生活质量。Objective:To analyze the application effect of superior thyroid pole vascular dissection through cricothyroid space approach in thyroidectomy.Methods:Eighty patients treated with thyroidectomy in Binzhou Central Hospital from June 2021 to June 2022 were selected as the study subjects.They were divided into control group and experimental group according to random number table method,with 40 cases in each group.The control group used the sternohyoid muscle-sternothyroid muscle space approach and the experimental group used the cricothyroid space approach for superior thyroid pole vascular dissection.Treatment satisfaction,complications,intraoperative superior laryngeal nerve search time,and quality of life were compared between the two groups.Results:The total treatment satisfaction in the experimental group was higher than that in the control group,and the difference was statistically significant(P=0.013).The total incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P=0.043).The intraoperative superior laryngeal nerve search time in the experimental group was shorter than that in the control group,and the difference was statistically significant(P=0.036).Before surgery,there was no significant difference in the scores of social function,emotional function,physical pain,physiological function and overall health between the two groups(P>0.05).After surgery,the scores of social function,emotional function,physical pain,physiological function and overall health in the two groups were higher than those before surgery,the scores in the experimental group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The superior thyroid pole vascular dissection through cricothyroid space approach in thyroidectomy has a significant application effect,and can shorten the intraoperative superior laryngeal nerve search time,reduce the incidence of complications,and improve the treatment
