摘 要:目的观察弱精子症(AS)患者精浆中转化生长因子-β1(TGF-β1)、白介素-17(IL-17)、白介素-6(IL-6)和精子顶体酶活性水平,探讨精浆TGF-β1、IL-17和IL-6水平对AS及AS患者精子顶体酶活性的影响。方法选取2022年6月至12月广州医科大学附属番禺中心医院诊治的110例AS患者作为研究对象,设为观察组。另选取同期51例健康体检且精液检查正常的男性设为对照组。根据前向运动精子百分率(PR)将观察组进一步分为A组、B组和C组。检测各组的精液常规参数、精子顶体酶活性及精浆TGF-β1、IL-17和IL-6水平,采用Logistic回归分析AS发生的影响因素,采用线性回归分析AS患者精子顶体酶活性的影响因素。结果与对照组比较,A组、B组、C组精浆TGF-β1、IL-17及IL-6水平显著升高(P<0.05)。精浆TGF-β1、IL-17和IL-6水平与PR、精子总活力[PR+非前向运动精子百分率(NP)]、精子顶体酶活性呈负相关(P<0.01);精浆IL-17水平与精液pH、精液体积、精子总数呈负相关(P<0.05);精浆IL-6水平与精子总数呈负相关(P<0.05)。精浆TGF-β1、IL-17和IL-6水平与AS的发生具有相关性(P<0.01),精浆IL-17和IL-6水平升高导致精子顶体酶活性降低(P<0.01)。结论精浆中TGF-β1、IL-17和IL-6水平升高是AS发生的危险因素,AS患者精浆IL-17和IL-6水平升高是导致精子顶体酶活性降低的影响因素。Objective To observe the levels of transforming growth factor-β1(TGF-β1),interleukin-17(IL-17),interleukin-6(IL-6)and sperm acrosomal enzyme activity in the seminal plasma of patients with asthenospermia(AS),and to investigate the effects of seminal TGF-β1,IL-17 and IL-6 levels on AS and acrosomal enzyme activity in patients with AS.Methods A total of 110 patients with AS admitted to the Affiliated Panyu Central Hospital of Guangzhou Medical University from June to December 2022 were enrolled as study objects and set as the observation group.Another 51 healthy males participated in a health examination and had a normal semen examination were enrolled as the control group.The AS group was further divided into group A,group B and group C according to the percentage of progressive motility(PR).Semen routine parameters,acrosomal enzyme activity,seminal plasma TGF-β1,IL-17 and IL-6 levels were analyzed.Logistic regression was used to analyze the risk factors for AS,and linear regression was used to analyze the influencing factors of sperm acrosomal enzyme activity.Results Compared with the control group,seminal plasma levels of TGF-β1,IL-17 and IL-6 of group A,group B and group C were significantly increased(P<0.05).Seminal levels of TGF-β1,IL-17,and IL-6 were negatively correlated with PR,total sperm motility[PR+percentage of nonprogressive motility(NP)],and sperm acrosomal enzyme activity(P<0.01).Seminal level of IL-17 was negatively correlated with semen pH,semen volume and total sperm count(P<0.05).Seminal plasma level of IL-6 was negatively correlated with total sperm count(P<0.05).Levels of seminal plasma TGF-β1,IL-17 and IL-6 were associated with the occurrence of AS(P<0.01).The increased seminal plasma levels of IL-17 and IL-6 lead to the decrease of acrosomal enzyme activity(P<0.01).Conclusions The increased levels of TGF-β1,IL-17 and IL-6 in seminal plasma are risk factors for the occurrence of AS,and the increased levels of seminal plasma IL-17 and IL-6 in patients with AS are influencing factors le
