血清可溶性内皮糖蛋白水平与复发性流产女性早期妊娠流产的相关性

摘　　要：目的探讨血清可溶性内皮糖蛋白(sEng)与复发性流产(RSA)女性早期妊娠流产的相关性。方法纳入2021年1月至2023年1月于海南省妇女儿童医学中心妇产科门诊就诊的197例RSA早孕期女性作为RSA组,98例同期同一医院就诊的正常早孕期女性作为正常早孕期组。随访妊娠13^(+6)周之前的妊娠结局,将RSA组分为RSA流产组(n=66)和RSA继续妊娠组(n=131)。比较三组血清sEng水平,并分析其与RSA女性早期妊娠流产的相关性。结果正常早孕期组女性妊娠13^(+6)周之前流产率低于RSA组(P<0.05);RSA流产组女性D-二聚体(D-D)、白介素-2(IL-2)、肿瘤坏死因子α(TNF-α)、sEng水平均高于RSA继续妊娠组、正常早孕期组,且RSA继续妊娠组以上指标均高于正常早孕期组(P<0.05);Pearson直线相关显示,RSA组女性血清sEng与D-D、IL-2、TNF-α水平均呈正相关(r>0,P<0.05);Logistic回归分析显示,无论是否混杂其他因素,血清sEng水平与RSA女性早期妊娠流产存在关联性(P<0.05)。结论RSA女性血清sEng水平异常升高可能与早期妊娠流产有关,涉及的机制可能包括内皮细胞损伤、炎症、凝血功能异常等。Objective To investigate the correlation between serum soluble endoglin(sEng)and early pregnancy abortion in women with recurrent spontaneous abortion(RSA).Methods A total of 197 RSA early pregnancy women who visited the Department of Obstetrics and Gynecology Hainan Women and Children's Medical Center from January 2021 to January 2023 were included as the RSA group,and 98 normal early pregnancy women who visited the same hospital during the same period were included as the normal early pregnancy group.The pregnancy outcomes before 13 weeks^(+6)of pregnancy were followed-up,and RSA group was divided into the RSA abortion group(n=66)and the RSA continued pregnancy group(n=131).The serum sEng levels of the three groups were compared and their correlation with early pregnancy abortion was analyzed.Results The abortion rate before 13^(+6)weeks in normal early pregnancy women was lower than RSA early pregnancy women(P<0.05).The levels of D-dimer(D-D),interleukin-2(IL-2),tumor necrosis factorα(TNF-α),and sEng in the RSA abortion group were higher than those in the RSA continued pregnancy group and normal early pregnancy group,and the RSA continued pregnancy group was higher than that in the normal early pregnancy group(P<0.05).Pearson linear correlation showed that serum sEng was positively correlated with D-D,IL-2,and TNF-α in RSA group(r>0,P<0.05).Logistic regression analysis showed that serum sEng level was associated with early pregnancy abortion in RSA women regardless of other factors mixed(P<0.05).Conclusions The abnormal increase of serum sEng levels in RSA women may be related to early pregnancy abortion.The involved mechanisms may include endothelial cell damage,inflammation,and coagulation dysfunction.
