摘 要:充血是心力衰竭(心衰)患者几乎同时发生的结构和功能状态的改变,它可能是由过度的神经激素激活引起的,并可表现为外周水肿和肺水肿,是影响心衰患者预后的重要因素之一。因此,充血的识别是诊断心衰前状态最重要的一点。本综述就目前实验数据,对有关心衰充血的传统及新型评估方法进行了归纳,客观分析了不同评估手段的优缺点,重点阐述了估计的血浆容量(ePVS)与临床充血和预后相关的证据,为临床心衰患者充血评估及干预策略提供新的诊疗思路。Congestion is an almost simultaneous structural and functional change in patients with heart failure(HF).It may be caused by excessive neurohormone activation and can be manifested as peripheral edema and pulmonary edema,and it is one of the important factors affecting the prognosis of HF patients.Therefore,the identification of congestion is the most important point in the diagnosis of pre-heart failure state.Based on the current experimental data,this review summarizes the traditional and new evaluation methods of congestive heart failure,objectively analyzes the advantages and disadvantages of different evaluation methods,focusing on the evidence that ePVS is related to clinical hyperemia and prognosis,so as to provide new ideas for diagnosis and treatment as well as intervention strategies for congestive heart failure patients.
