摘  要:目的探讨两种不同材质(聚丙烯、复合)补片修补对腹壁疝大鼠腹横筋膜组织转化生长因子-β1(TGF-β1)及Collagen合成代谢的影响及作用。方法选取8周龄,SPF级雄性SD大鼠随机分为4组,即正常对照组、腹壁疝组、聚丙烯补片修补组、复合补片修补组,每组6只。大鼠下腹部以钝性分离皮肤与肌肉组织后切除3 cm×2 cm大小腹壁肌肉筋膜组织来造腹壁缺损(AH)模型,并采用聚丙烯补片(PM)、复合补片(CM)材质补片进行肌筋膜前置(Onlay)方式修补;4组大鼠术后均饲养8周后处死,取出腹壁全层(补片及腹横筋膜)组织,加入TRIzol裂解液,取总RNA;采用RT-PCR实验方法检测TGF-β1、Smad2/3、胶原蛋白(CollagenⅠ型和Ⅲ型)及基质金属蛋白酶(MMP-2、MMP-9)的mRNA表达水平。结果AH大鼠显著降低腹横筋膜组织TGF-β1、Smad-2以及Smad-3的mRNA表达水平,与正常对照组相比差异有统计学意义(P<0.001);PM及CM修补均有效的增加TGF-β1、Smad-2以及Smad-3的mRNA水平(P<0.01);而复合补片组明显增高mRNA表达水平,与聚丙烯补片组相比,差异有统计学意义(P<0.05)。AH大鼠显著降低腹横筋膜组织中CollagenⅠ和CollagenⅢ的mRNA表达水平(P<0.001);复合补片组更有效的增高Collagen(Ⅰ型和Ⅲ型)的mRNA表达水平,与聚丙烯补片组相比,差异有统计学意义(P<0.05)。AH大鼠显著增高腹横筋膜组织中MMP-2和MMP-9的mRNA表达水平(P<0.001);复合补片组更有效的降低MMP-2和MMP-9的mRNA表达水平,与聚丙烯补片组相比,差异有统计学意义(P<0.05)。结论聚丙烯补片和复合补片修补均可以改善AH大鼠腹横筋膜组织TGF-β1及Collagen的合成,而复合补片更加有效的促进胶原蛋白的合成。Objective To explore the effects of two different materials(polypropylene,composite)meshes on transforming growth factor-beta1(TGF-β1)and Collagen anabolism of abdominal transverse fascia in rats with abdominal wall hernia.Methods Eight-week-old male SPF grade SD rats were randomly divided into four groups:control group(Control),abdominal wall hernia(AH),polypropylene mesh repair(PM)and composite mesh repair(CM)group,with 6 rats in each group.The lower abdomen of rats was cut after blunt separation of skin and muscle tissue.The model of abdominal wall defect was made by removing 3 cm×2 cm full thickness abdominal wall tissue,and was repaired with PM and CM mesh materials using Onlay mode respectively.The rats in the four groups were killed after 8 weeks of feeding.The whole layer of abdominal wall(mesh and transverse fascia)was taken out,added to TRIzol lysate buffer,and total RNA was taken;the RT-PCR method was used to detect the mRNA expression levels of TGF-β1,Smad-2,Smad-3,CollagenⅠandⅢ,and matrix metalloproteinase(MMP-2,MMP-9).Results Compared to control group,the mRNA expression levels of TGF-β1,Smad-2 and Smad-3 in transverse abdominal fascia of AH rats were significantly decreased(P<0.001);Both PM and CM groups can effectively increase the levels of TGF-β1 and Smad-2/3 mRNA expressions(P<0.01),and CM group had significantly higher mRNA expression than PM group(P<0.05).AH rats significantly decreased the mRNA expression of CollagenⅠand CollagenⅢin the abdominal transverse fascia(P<0.001);The CM group was more effective in increasing the mRNA expression level of Collagen(typeⅠandⅢ)than the PM group,and the difference was significant(P<0.05).AH rats significantly increased the mRNA expression levels of MMP-2 and MMP-9 in the abdominal transverse fascia(P<0.001);The mRNA expression levels of MMP-2 and MMP-9 in the CM group were significantly lower than those in the PM group(P<0.05).Conclusion The polypropylene mesh and composite mesh could improve the TGF-β1 and Collagen anabolism of transve
