从“伏风引触”论治特应性皮炎

摘　　要：特应性皮炎是一种临床常见的慢性、复发性、炎症性皮肤病,其发病与体质密切相关。中医学认为,“伏风引触”是特应性皮炎发病的主要原因。特应性皮炎患者禀受先天邪毒,又因正气欠充,邪气得伏,深藏血脉,每逢外邪引触,外发肌肤,因此皮损发作迅速,范围广泛。风邪内生、因虚而伏是特应性皮炎的发病基础,因加而发、内外合邪是特应性皮炎发作期的主要病机,邪气久稽又暗耗津血,化燥伤阴,形成恶性循环。特应性皮炎患者临床常见表里同病,虚实夹杂,常反复,难治愈,临证应抓住“伏风”与“正虚”这两处关键,运用“补”“透”相兼之法,一方面宣通水道,透邪外出,一方面补气养血,托邪达外,并根据病情变化分期论治,临床验之效佳。Atopic dermatitis is a common chronic,recurrent and inflammatory skin disease in clinical practice,and its onset is closely related to physical constitution.Traditional Chinese medicine believes that"Fufeng induced stimulation"is the main cause of atopic dermatitis.Patients with atopic dermatitis are naturally aflicted with congenital toxins,but due to insufficient positive energy,the evil energy accumulates and hides deep in the bloodstream.Whenever external pathogens touch,they spread to the skin,resulting in rapid and widespread skin lesions.The pathogenesis of atopic dermatitis is based on the endogenous generation of wind evil and the accumulation of yin due to deficiency.The main pathogenesis during the onset of atopic dermatitis is the combination of internal and external evil.Prolonged accumulation of evil qi can consume the body's blood,transform dryness and damage yin,forming a vicious cycle.Patients with atopic dermatitis often have the same disease both on the surface and inside,with a mixture of deficiency and excess,often attacking repeatedly and being difficult to cure.In clinical practice,it is important to focus on the key points of"Fufeng suppression"and"deficiency correction",and use a combination of"tonifying"and"penetrating"methods.On the one hand,it can promote the flow of water,expel evil,and on the other hand,it can nourish qi and blood,promote evil to the outside.Treatment should be staged according to changes in the condition,and the clinical trial results are excellent.
