摘　　要：抽动-秽语综合征(TS)是抽动障碍(TD)中较为严重的一种形式,以头面部、肢体或躯干的多发性肌肉抽动与暴发性不自主发声为主要特征,属“肝风”“慢惊风”“瘈疭”等范畴。马丙祥教授认为气机逆乱则内风窜行引发抽动,风痰鼓动阻遏咽喉则发声频作,内陷蒙蔽心窍则秽语不识人,经多年临证经验,总结提出“气运痰消动自制”的见解,指出在TS治疗方面应结合儿童“三有余,四不足”的生理特点,以“畅肺气、和肝脾、清心窍、壮精气”为主要治疗手段,梳理气机以除痰熄风。Tourette syndrome(TS)is a tic disorder(TD)in a more serious form,mainly characterized by sudden involuntary multiple muscle tics in the head,face,limbs,or trunk,and vocalizations,which belongs to the categories of"liver wind"or"chronic infantile convulsion".Professor Ma Bingxiang believes that when qi movement becomes deranged,internal wind arises and gives rise to muscle tic,when wind phlegm obstructs the throat,it can causes vocal tic,and if wind phlegm moves inward and block the heart orifice,coprolalia may occur and the person may not be able to recognize people.Professor Ma,after years of clinical experience,has summarized and put forward the view of"Unrest is resolved when phlegm is eliminated and qi movement becomes normal".He pointed out that in the treatment of TS,one should combine the physiological characteristics of children's"three surpluses and four deficiencies",and take promoting lung qi,harmonizing liver and spleen,clearing the heart and orifices,and strengthening essence qi as the main method to sort out qi movement.This way,the phlegm can be eliminated and wind can be extinguished.
