CT下过敏性鼻炎患者翼管外口横截面积测量及其临床意义

摘  要:目的 探讨过敏性鼻炎(allergic rhinitis, AR)、AR伴鼻窦炎、AR伴鼻窦炎及鼻息肉患者高分辨CT下翼管外口横截面积的变化差异及临床意义。方法 随机选取辽宁省人民医院耳鼻喉科2018年10月至2022年5月的80名患者,对照组(无过敏性鼻炎)20例与过敏性鼻炎组(60例)。80名患者共分为4组:对照组(Ⅰ组)20例、单纯过敏性鼻炎组(Ⅱ组)20例、过敏性鼻炎伴鼻窦炎组(Ⅲ组)20例、过敏性鼻炎伴鼻窦炎及鼻息肉组(Ⅳ组)20例。通过高分辨鼻窦冠状位CT对翼管外口进行定位,通过PASS软件系统对翼管外口进行类型分类,以及对翼管横截面积及双侧外口距离进行影像学测量,并对测量结果进行统计学分析。结果 (1)翼管外口在鼻窦冠状面呈圆形或扁圆形,口径大小不等,按照翼管分型方法,进行分型计数,统计学分析χ^(2)=2.470,P>0.05,各组翼管分型差异无统计学意义;(2)利用影像诊断工作站中多边形工具,进行翼管外口面积测量。4组平均翼管外口面积的q检验显示Ⅲ组、Ⅳ组与Ⅰ组翼管外口面积比较均差异有统计学意义(P<0.05);Ⅱ组与Ⅰ组比较,q=2.75,P>0.05,差异无统计学意义;Ⅳ组与Ⅲ组比较q=3.38,P<0.05,差异有统计学意义。而Ⅳ组出现翼管外口面积的增大的趋势最为明显;Ⅱ组、Ⅲ组、Ⅳ组与Ⅰ组翼管外口距离比较均差异有统计学意义(P<0.05):Ⅱ组与Ⅰ组比较,q=3.03,P<0.05;Ⅲ组与Ⅰ组比较,q=4.83,P<0.01;Ⅳ组与Ⅰ组比较,q=7.09,P<0.01;而Ⅳ组与Ⅲ组比较q=2.25,P>0.05,差异无统计学意义。结论 过敏性鼻炎(AR)、AR伴鼻窦炎、AR伴鼻窦炎及鼻息肉患者在高分辨CT下,翼管外口之间距离、翼管外口横截面积有差异,而AR伴鼻窦炎及鼻息肉患者在高分辨CT下翼管横截面积影响最大,有扩大的趋势,为翼管周围的手术提供解剖参考。Objective To investigate the differences and clinical significance of changes in the cross-sectional area of the external pterygoid canal under high-resolution CT in patients with allergic rhinitis(AR),AR with sinusitis,AR with sinusitis and nasal polyps.Methods 80 patients including normal control group(20 patients)and allergic rhinitis patients(60 patients)in our department were randomly selected from October 2018 to May 2022.80 patients were divided into 4 groups with 20 cases in each group:normal group(groupⅠ),simple allergic rhinitis group(groupⅡ),allergic rhinitis with sinusitis group(group Ⅲ),and the allergic rhinitis with sinusitis and nasal polyps group(group Ⅳ).The anterior opening of the vidian canal was located by high resolution sinus coronal CT,and the anterior opening of the vidian canal was classified by PASS software system.The distance and cross-sectional area of the anterior opening of both vidian canal were measured by imaging tools,and the measurement results were statistically analyzed.Results(1)The pterygoid ducts were round or oblate in the coronal plane of the sinus,with varying calibre sizes,and were typed and counted according to the pterygoid duct typing method,with no significant difference in pterygoid duct typing between groups by statistical analysisχ^(2)=2.470,P>0.05.(2)Using the polygon tool in the diagnostic imaging workstation,measurement of the anterior pterygoid canal area was performed.The q-test of the mean pterygoid canal external orifice area in the four groups showed that compared with group I,there were significant differences in groupsⅢandⅣ(P<0.05);compared with group Ⅰ,there were no significant differences in group Ⅱ(q=2.75,P>0.05);compared with Ⅲ,there were significant differences in group Ⅳ(q=3.38,P<0.05).The trend of increasing the area of the anterior pterygoid canal was the most obvious in group IV;there was a significant difference(P<0.05)in the comparison of VC distance between groupⅡ,groupⅢand groupⅣand groupⅠ;groupⅡwas compared
