摘 要:目的探讨血液常规检测指标与孕妇妊娠期糖尿病(GDM)发病的相关性。方法采用前瞻性队列研究,随机选择2016年11月—2018年12月在广州市妇女儿童医疗中心开展常规产检以及分娩的1775例孕妇作为研究对象,收集孕妇的产检信息及血液常规检测结果。结果1775例孕妇中,GDM发病率为15.38%。多因素logistic回归分析结果显示:在孕早期和孕晚期,红细胞计数、白细胞计数是GDM发生的影响因素。受试者工作特征(ROC)曲线显示:联合孕早期红细胞计数、白细胞计数、孕妇年龄及孕前体质量指数(BMI)预测GDM的曲线下面积(AUC)最大,为0.828(95%CI:0.776~0.880),灵敏度和特异度分别为75.40%及80.10%。结论孕早期红细胞计数和白细胞计数升高与孕妇GDM发生有一定相关性,可能作为GDM发病的早期预测因子。Objective To explore the correlations between routine blood test indicators and occurrence of gestational diabetes mellitus(GDM).Methods A prospective cohort study was conducted involving 1775 pregnant women receiving routine prenatal examination and giving birth to their babies in Guangzhou Women and Children's Medical Centre from November 2016 to December 2018.The prenatal records and routine blood test results were collected.Results The incidence rate of GDM was 15.38%among 1775 pregnant women.Multivariable logistic regression analysis showed that red blood cell count and white blood cell count were influencing factors of GDM in early and later pregnancy.Receiver operating characteristic(ROC)curve showed that the area under ROC curve of combined detection of red blood cell count,white blood cell count,maternal age,and body mass index(BMI)before pregnancy in early pregnancy in predicting GDM was the highest,which was 0.828(95%CI:0.776-0.880),the sensitivity and specificity were 75.40%and 80.10%,respectively.Conclusion High red blood cell count and white blood cell count in early pregnancy are correlated with the occurrence of GDM,which may be early predictors of GDM.
