摘　　要：目的探讨子宫内膜异位症患者外周血调节性T细胞(Treg)上免疫检查点分子T细胞免疫球蛋白和免疫受体酪氨酸抑制基序(TIGIT)和CD226的表达和价值。方法以健康检查者为对照,收集2022年7—12月在复旦大学附属妇产科医院诊断为子宫内膜异位症的患者外周血(疾病组)。采用流式细胞术检测抗凝外周血中Treg及细胞上表面分子CD226和TIGIT的百分含量,并比较两组间的差异。结果子宫内膜异位症患者外周血中的Treg细胞占CD4+T细胞的百分含量为(7.75±1.94)%,对照组中Treg细胞占CD4+T细胞的百分含量为(7.15±1.78)%,差异无统计学意义(P>0.05);TIGIT+Treg细胞在疾病组中的比例为(56.10±7.95)%,在对照组中的比例为(49.94±6.45)%,差异有统计学意义(P<0.01);CD226+Treg细胞在疾病组中的比例为(46.67±6.96)%,其在对照组中的表达比例为(47.31±6.20)%,差异无统计学意义(P>0.05)。结论外周血Treg细胞的数量没有显著升高,免疫检查点分子TIGIT在子宫内膜异位症患者外周血Treg细胞上的表达异常升高,而CD226无明显表达异常;TIGIT的异常表达与子宫内膜异位症的发生可能相关,或可用于临床辅助诊断子宫内膜异位症的发生。Objective To investigate the expression and value of the immune checkpoint molecules T cell immunoglobulin and immunoreceptor tyrosine-based inhibitory motif domain(TIGIT)and CD226 on peripheral blood regulatory T cell(Treg)in endometriosis patients.Methods Peripheral blood was collected from patients diagnosed with endometriosis at the Obstetrics and Gynaecology Hospital of Fudan University(Endometriosis group),using healthy participants as controls.The percentage of Treg and surface molecules CD226 and TIGIT in the anticoagulated peripheral blood was measured by flow cytometry and compared between the two groups.Results The percentage of Treg cells in the peripheral blood of EM patients was(7.75±1.94)%,compared with(7.15±1.78)%in the control group,with no statistically significant difference(P>0.05);the percentage of TIGIT+Treg cells in the Endometriosis group was(56.10±7.95)%,which was significantly higher than that of(49.94±6.45)%in the control group,with a statistically significant difference(P<0.01);the proportion of CD226+Treg cells in the endometriosis group was(46.67±6.96)%and their expression in the control group was(47.31±6.20)%,and the difference was not statistically significant(P>0.05).Conclusion The proportion of peripheral blood Treg cells was not significantly elevated,whereas the immune checkpoint molecule TIGIT had abnormally elevated expression on peripheral Treg cells in endometriosis patients,while CD226 had no significant abnormal expression;abnormal expression of TIGIT may be associated with the pathogenesis of endometriosis and may be used as a clinical adjunct to diagnose endometriosis.
