摘 要:目的探讨活血起萎颗粒对精索静脉曲张(VC)合并勃起功能障碍(ED)大鼠eNOS、nNOS和Cx43的影响,明确活血起萎颗粒治疗VC合并ED的作用机制。方法将120只SD大鼠分别分为正常组、假手术组、模型组、活血起萎颗粒低、中、高剂量组。通过Turner法和APO试验,建立VC合并ED大鼠模型。第1、3、8周末进行APO试验检测勃起功能;测量干预前后左侧精索静脉直径;ELISA检测性激素水平;透射电镜观察阴茎结构;RT⁃PCR检测阴茎nNOS、eNOS mRNA的表达;Western blot检测阴茎组织Cx43的表达。结果造模后,模型组、各治疗组左侧精索静脉宽度显著增加(P<0.01);干预后,各治疗组静脉宽度降低(P<0.05)。第1次,APO试验,各组大鼠勃起功能无差异(P>0.05);第2次,模型组、各治疗组勃起功能显著下降(P<0.01);第3次,各治疗组勃起功能恢复(P<0.05)。模型组FSH、LH水平升高(P<0.05),TT、FT水平降低(P<0.05);中、高剂量组血清FSH、LH水平下降(P<0.05),TT、FT水平升高(P<0.05)。模型组阴茎eNOS、nNOS mRNA表达显著下调(P<0.01);各治疗组阴茎eNOS、nNOS mRNA表达显著上调(P<0.01)。模型组Cx43蛋白量显著下调(P<0.01);各治疗组Cx43蛋白量显著上调(P<0.01)。各治疗组CCSMC结构完整,少量视野胶原纤维结构恢复,线粒体恢复,内质网数量增加。结论活血起萎颗粒可改善精索静脉曲张,并改善相关性激素水平,使阴茎海绵体中eNOS、nNOS表达上调和Cx43表达上调,进而改善大鼠的勃起功能。Objective To investigate the effects of Huoxueqiwei granules on the expressions of eNOS,nNOS and Cx43 in the rats with VC and ED,and explore its mechanism in the treatment of VC and ED.Methods A total of 120 SD rats were divided into the normal group,the sham group,the model group,and low⁃,medium⁃,and high⁃dose Huoxueqiwei granule groups.A rat model of VC and ED was established through Turner’s method and APO test.APO tests were performed to detect erectile function on the 1st,3rd,and 8th weekend;the diameter of the left spermatic vein was measured before and after intervention;Levels of serum FSH,LH,FT,and TT were detected by ELISA;The ultrastructure of penile tissue was observed using a transmission electron microscope;The mRNA expressions of nNOS and eNOS in penile were analyzed by RT⁃PCR;The expression of Cx43 in penile was detected by western blot.Results After modeling,the width of the left spermatic vein in the model group and each treatment group was increased significantly(P<0.01).After intervention,the vein width in each treatment group was decreased(P<0.05).For the first APO test,there was no difference in the erectile function of rats in each group(P>0.05).For the second APO test,the erectile function of rats in the model group and each treatment group was decreased significantly(P<0.01).For the third APO test,the erectile function of rats in each treatment group was recovered(P<0.05).The levels of FSH and LH in the model group were increased(P<0.05),while the levels of TT and FT were decreased(P<0.05).Levels of serum FSH and LH in the medium and high dose groups were all decreased(P<0.05),while levels of TT and FT were increased(P<0.05).The mRNA expressions of eNOS and nNOS in penis were significantly downregulated(P<0.01)in the model group.The mRNA expressions of eNOS and nNOS in the penis of each treatment group were significantly upregulated(P<0.01).Cx43 protein level was significantly decreased(P<0.01)in the model group,and its level in each treatment group was significantly increased(P
