基于经筋理论探讨冀来喜教授针刀分刺枕下肌群治疗颈性头痛经验撷菁

摘　　要：文章总结归纳冀来喜教授基于经筋理论运用针刀分刺枕下肌群治疗颈性头痛的诊疗思路及操作要点。冀来喜教授作为全国名中医,在从事临床、教学工作的30余年间针药结合,中西医汇融,强调在审清证候的前提下选取各种针具杂合而组,临床经验丰富,建立起以“新九针”为主的针灸诊疗体系。冀来喜教授认为颈性头痛“痛在头而病在颈”,造成颈性头痛的根本原因在于不良姿势及寒湿等外邪刺激导致颈枕部肌群的高张力和颈椎结构失稳,颈部生物力学失衡,进而造成位于颈枕部的神经和交感神经节兴奋,血管收缩,并在慢性刺激下形成无菌性炎症,形成粘连、瘢痕等病理变化。冀来喜教授基于解剖生理与经筋理论,整体论治,以颈治头,提出在经筋理论的指导下针刀分刺枕下肌群治疗颈性头痛的诊疗方案。针刀形似针,实为刀,既能激发全身经气,又能剥离松解粘连组织,以分刺手法从枕下肌群入手,直达肌肉间隙。整体论治,从颈治头,从经治头,通过松解颈枕部紧张肌群解除周围肌群高张力、恢复神经血管正常生理功能与组织代谢,促进炎症因子吸收,恢复头面部血供及正常生理状态。文章旨在阐释针刀分刺枕下肌群治疗颈性头痛的理论依据及诊疗经验,并结合典型病案,为针灸治疗颈性头痛提供系统规范的诊疗方案。The diagnosis and treatment ideas and operation points of Professor JI Laixi’s acupotomization of suboccipital muscle group in the treatment of neck headache based on the theory of meridian tendons is summarized in this paper.Professor JI Laixi believes that neck headache“pain in the head but the disease in the neck”,because of the high tension of the cervical occipital muscle group caused by neck biomechanical imbalance.Based on the theory of meridian tendons,Professor JI Laixi proposed the diagnosis and treatment plan of dividing the suboccipital muscle group by needle-knife,and treating it from a holistic perspective to relieve the hypertension of muscle group and restore the normal physiological function of neurovascular and tissue metabolism.The purpose of this paper is to explain the theoretical basis and diagnosis and treatment experience of acupotomization of suboccipital muscle group in the treatment of neck headache,and to provide systematic and standardized diagnosis and treatment plan for the treatment of neck headache by acupuncture with typical medical records.
