梁惠卿教授基于“邪之所在,皆为不足”理论治疗乙型肝炎肝纤维化的学术思想

摘　　要：乙型病毒性肝炎(简称乙型肝炎)肝纤维化为慢性乙型肝炎进展过程中的一个重要病理阶段,临床表现并无特异性,但其在整个疾病过程中的重要地位不容置疑。它是肝硬化形成的必经之路,也是影响慢性乙型肝炎转归预后的重要因素。同时,基于中医“未病先防,已病防变”的重要理论,对临床中关于乙型肝炎肝纤维化的诊疗提出了更高的要求。“邪之所在,皆为不足”这一中医经典理论认为外邪之所以入侵人体皆是由于正气亏虚不足以抵抗外邪。乙型肝炎肝纤维化正是由于人体正气亏虚,毒邪趁虚而入并久居不去,壅于肝脏,肝络失和,气机疏泄失常,气血失于运化,最终气虚血瘀,有形实邪产生发为此病。梁惠卿教授从这一理论出发,细探病机,从气虚血瘀的角度切入,寻药组方,研制皂术抗纤方,取其益气扶正、活血化瘀的功效提高机体抵抗力、逐步消解体内有形实邪,从源头逆转乙型肝炎肝纤维化的进展。经过不断的临床实践及优化,该方在临床应用中屡获良效。文章从“邪之所在,皆为不足”角度探讨皂术抗纤方的组方思路及抗乙型肝炎肝纤维化的机制,旨在进一步挖掘梁教授基于该理论治疗乙型肝炎肝纤维化的学术思想,为临床提供更多参考和启发。Hepatitis B liver fibrosis is an important pathological stage in the progression of chronic hepatitis B,and the clinical manifestations are not specific,but its important position in the whole disease process cannot be ignored.It is the way for cirrhosis to develop,and it is also an important factor affecting the prognosis of chronic viral hepatitis B.At the same time,based on the important theory of traditional Chinese medicine“prevention before disease onset,and prevent change after disease”,higher requirements are put forward for the diagnosis and treatment of hepatitis Bliver fibrosis in clinical practice.The classic theory of Chinese medicine that“Wherever the disease is,it is due to insufficient vital Qi(气)”holds that the reason why external evil invades the human body is due to the deficiency of body resistance and is not enough to resist external evil.Hepatitis Bliver fibrosis is due to the deficiency of vital Qi in the human body,evil Qi takes advantage of the deficiency to get in and stay for a long time,trapped in the liver,liver collateral is out of harmony,Qi movement is disorder,Qi and blood are lost in transportation,and finally Qi deficiency and blood stasis,real evil is produced,and this disease occurs.Starting from this theory,Professor LIANG Huiqing carefully explored the pathogenesis,began from the perspective of Qi deficiency and blood stasis,sought prescriptions and medicines,developed the Zaozhu Kangxian prescription(皂术抗纤方),and took its effects of invigorating Qi and supporting vital Qi,activating blood circulation and removing stasis,resisting the invasion of evil,and gradually dissolving the tangible evil that has formed in the body,and reversing the progress of hepatitis Bliver fibrosis from the source.After continuous clinical practice and optimization,it has repeatedly achieved good results in clinical applications.This article discuss the mechanism of the Zaozhu Kangxian prescription against hepatitis B,and liver fibrosis from the perspective of“where the evil is
