基于文献计量学及可视化技术的中医药治疗溃疡性结肠炎研究现状及热点分析

摘　　要：目的:全面分析中医药治疗溃疡性结肠炎的临床研究现状及热点。方法:通过检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库等获取中医药治疗溃疡性结肠炎的临床研究文献,文献日期截至2023年3月31日。利用NoteExpress 3.0软件对文献进行筛查并建立相关数据库,同时基于文献计量学和统计学方法搜集所筛选文献的基本信息(包括发文时间、作者、出产机构和关键词等)。使用Citespace 6.2软件对中医药治疗溃疡性结肠炎开展热点分析。结果:①依据纳入与排除标准筛选后最终获得以临床研究为主的合格文献2160篇。1989年-2023年3月发文量整体呈上升趋势,2014年以后的发文量上涨幅度较之前更大,并多刊载于专业性较强的期刊。中医药治疗溃疡性结肠炎的研究呈现出质量与数量齐头并进的趋势。②以朱叶珊、何芳、刘世举、杨杰等为代表的临床研究团队影响力较强。研究机构相对集中,主要为江苏、河南和辽宁等地的中医药大学及其附属机构,但区域间合作程度较低。③对纳入文献的关键词进行共现和聚类分析后发现,溃疡性结肠炎、临床疗效、美沙拉嗪、中药灌肠、炎性因子等13个关键词出现频次多且中心性高。④对关键词时线图和突现图进行分析,发现肠道菌群、氧化应激、湿热内蕴证、不良反应等为近几年的研究热点。⑤经过分析文献可知,溃疡性结肠炎的干预措施虽多,但中药与西药联用是目前最为常见的治疗方法。结论:中医药治疗溃疡性结肠炎的临床研究虽然取得了长足的进步,但目前作用机制仍不明确;湿热内蕴型是目前溃疡性结肠炎研究的主要证型,而中西医结合则是治疗溃疡性结肠炎的优选方案,其免疫调控、抗氧化及肠道菌群平衡的相关作用机制与靶点研究是未来探索的主要领域,实现跨区域联动合作与交流是推动溃疡性�Objective:To analyze the current status and hotspots of clinical research on traditional Chinese medicine for the treatment of ulcerative colitis.Methods:Clinical research literature on TCM treatment of ulcerative colitis was obtained by searching databases such as China Knowledge(CNKI),Wanfang database(WF),VIP database(VIP),and China Biomedical Medicine Literature Database(CBM).The latest publication date of the literature was March 31,2023.Screening of literature and creation of relevant databases was performed by NoteExpress 3.0 software.At the same time,based on bibliometric and statistical methods,the basic information of the collected and screened literature(including the time of publication,authors,producing organizations and keywords,etc.)was extracted.A hotspot analysis of TCM treatment of ulcerative colitis was carried out using Citespace 6.2 software.Results:①Based on the inclusion and exclusion criteria,a total of 2160 articles were obtained from the literature.Between 1989 and March 2023,there is an overall upward trend in the number of articles issued.The rise in the number of publications after 2014 was greater than before,and the papers were mostly published in more specialized journals.Research on the treatment of UC with Chinese medicine has shown a trend of both quality and quantity.②The clinical research team of UC,represented by ZHU Yeshan,HE Fang,LIU Shiju,YANG Jie,has strong influence.Research institutions were relatively concentrated,mainly traditional Chinese medicine universities and their affiliates in Nanjing,Henan and Liaoning and other areas,but the degree of regional cooperation was low.③Co-occurrence and cluster analysis of keywords in the included literature revealed that 13 keywords such as UC,clinical efficacy,mesalazine,herbal enemas,and inflammatory factors appeared frequently and with high centrality.④Analysis of the keyword time line diagram and emergence diagram revealed that intestinal flora,oxidative stress,syndrome of damp and heat accumulation,and adverse reacti
