摘　　要：目的分析上海市静安区精神残疾人扩大免费服药政策的施行效果,探索适宜服务模式。方法通过数据收集、统计,回顾性分析政策施行8年,患者的一般人口学资料、经济支出变化;了解社会工作效果指标;对政策服务提供者和对象开展定性访谈。结果2013—2020年免费服药人数逐年增加;门诊总费用在2014年达到高峰,在2018年达到低谷,后又逐年缓慢增长;人均门诊费用与门诊总费用趋势相似;患者的规律服药率、规范管理率和显好率均有显著增长。结论在服务对象、申请流程等方面需予重点关注、及时调整;随着政策和互联网医院不断优化,该政策有较大的社区推广及落实空间,并可为其他省市的免费服药项目提供借鉴。Objective:To analyze the effect of expanding the policy of free medication for mental disabilities in Jing’an District of Shanghai and explore the appropriate service model.Method:The general demographic data and economic expenditure changes of patients in 8 years after the implementation of the policy were retrospectively analyzed through data collection and statistics.To understand the social work effectiveness indicators,the qualitative interviews were conducted.Result:From 2013 to 2020,the number of people taking medicine free of charge has increased year by year.The total outpatient expenses reached a peak in 2014,reached a trough in 2018,and then increased slowly year by year.The trend of per capita outpatient expenditure was similar to that of the total outpatient expenditure.And the rate of regular medication,standardized management and remarkable improvement of patients increased significantly.Conclusion:Key attention and timely adjustments are needed in terms of service recipients and application processes.With the continuous optimization of the policy and Internet hospitals,the policy has greater space for community promotion and implementation,and can provide references for free medication projects in other provinces and cities.
