摘 要:目的观察阿奇霉素注射液联合普米克令舒雾化吸入治疗新生儿肺炎(neonatal pneumonia,NP)的临床疗效。方法将140例NP患儿作为研究对象,用随机数字表法分成对照组(n=70,对症支持治疗联合阿奇霉素注射液治疗)与研究组(n=70,在对照组治疗的基础上联合普米克令舒雾化吸入治疗)。比较2组的临床疗效、心肌酶谱、血气分析指标、炎症因子水平及不良反应发生情况。结果研究组的总有效率(91.43%)高于对照组(78.57%),P<0.05。治疗后,2组心肌酶谱指标[肌酸激酶(creatine kinase,CK)/肌酸激酶同工酶(creatine kinase isoenzymes-MB,CK-MB)/乳酸脱氢酶(lactate dehydrogenase,LDH)]、动脉血二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))、各炎症因子[核转录因子-κB(nuclear transcription factor-κB,NF-κB)/降钙素原(procalcitonin,PCT)/白细胞介素-6(interleukin-6,IL-6)]水平较治疗前均降低(P<0.05);动脉血氧分压(partial pressure of oxygen,PaO_(2))较治疗前升高(P<0.05),且研究组的心肌酶谱指标(CK、CK-MB、LDH)、PaCO_(2)、各炎症因子(NF-κB、PCT、IL6)水平均较对照组降低(P<0.05),PaO_(2)较对照组升高(P<0.05)。研究组不良反应的总发生率为11.43%(8/70),与对照组[5.71%(4/70)]比较差异无统计学意义(χ^(2)=1.458,P=0.227)。结论阿奇霉素注射液联合普米克令舒雾化吸入可有效治疗NP,改善心肌酶谱,调节动脉血气,降低炎症因子水平,且安全性高。Objective To observe the clinical efficacy of Azithromycin Injections combined with Pulmicort Respules in treating neonatal pneumonia(NP).Methods A total of 140 children with NP were enrolled in this study,and randomly grouped into control group(n=70,symptomatic and supportive treatment with Azithromycin Injections)and study group(n=70,on the basis of the control group,combined with Pulmicort Respules aerosol inhalation therapy)using a random number table method.The clinical efficacy,myocardial enzyme spectrum,blood gas analysis indicators,inflammatory factor levels,and adverse reactions were compared between the 2 groups.Results The total effective rate of the study group(91.43%)was higher than that of the control group(78.57%),P<0.05.After treatment,the myocardial enzyme spectrum indexes[creatine kinase(CK)/creatine kinase isoenzymes-MB(CK-MB)/lactate dehydrogenase(LDH)],partial pressure of carbon dioxide(PaCO_(2)),and various inflammatory factors[nuclear transcription factor-κB(NF-κB)/procalcitonin(PCT)/interleukin-6(IL-6)]of the 2 groups were lower than before treatment(P<0.05).The partial pressure of oxygen(PaO_(2))was higher than before treatment(P<0.05),the myocardial enzyme spectrum indicators(CK,CK-MB,LDH),PaCO_(2),and various inflammatory factors(NF-κB,PCT,IL-6)in the study group were lower than those in the control group(P<0.05),and PaO_(2) was higher than that in the control group(P<0.05).The total incidence of adverse reactions in the study group was 11.43%(8/70),and there was no statistically significant difference compared with the control group[5.71%(4/70)](χ^(2)=1.458,P=0.227).Conclusion Azithromycin Injections combined with Pulmicort Respules aerosol inhalation therapy can effectively treat NP,improve myocardial enzyme spectrum,and regulate arterial blood gas,with high safety.
