宫腔镜电切手术与刮宫术在异常子宫出血患者中的应用效果及对其子宫内膜厚度影响

摘　　要：目的研究分析宫腔镜电切手术与刮宫术在异常子宫出血患者中的应用效果及对其子宫内膜厚度影响。方法随机选取2019年12月—2022年12月福建省平和县医院收治的异常子宫出血患者100例为研究对象,以随机数表法分成两组,各50例,其中电切组以宫腔镜电切手术治疗,刮宫组行刮宫术手术治疗。比较两组患者治疗前后血清指标、恢复指标,评价治疗效果及并发症。结果治疗后两组纤维蛋白原较治疗前显著提升,且电切组纤维蛋白原与刮宫组相比更高,两组性激素指标、活化部分凝血活酶时间、凝血酶原时间较治疗前下降,且电切组与刮宫组相比更低,差异有统计学意义(P均<0.05)。治疗后两组月经量、超声下子宫内膜厚度、经期疼痛程度、月经持续时间均较治疗前下降,电切组与刮宫组相比更低,而两组血清血红蛋白浓度、月经周期规律性较治疗前提升,电切组均高于刮宫组,差异有统计学意义(P均<0.05)。电切组治疗总有效率(98.00%)高于刮宫组(82.00%),并发症发生率(2.00%)低于刮宫组(16.00%),差异有统计学意义(χ^(2)=5.005、4.396,P均<0.05)。结论在异常子宫出血患者中,实施以手术治疗均可以获得较好疗效,而宫腔镜电切手术可明显改善血清性激素指标与凝血功能指标,减少月经量与子宫内膜厚度,减轻经期疼痛程度,缩短月经持续时间,改善月经周期规律与血清血红蛋白浓度,术后并发症少,效果理想。Objective To study and analyze the effect of hysteroscopic electrotomy and curettage on endometrial thickness in patients with abnormal uterine bleeding.Methods A total of 100 patients with abnormal uterine bleeding admitted to Pinghe County Hospital in Fujian Province from December 2019 to December 2022 were randomly selected as study objects,and were divided into two groups with 50 cases each by random number table method.The electrotomy group treated with hysteroscopic electrotomy and the curettage group was treated with curettage.Serum indexes and recovery indexes were compared between the two groups before and after treatment to evaluate the therapeutic effect and complications.Results The fibrinogen after treatment was significantly improved in two groups compared with before treatment,the fibrinogen of electrotomy group was higher than that of curettage group,the sex hormone indexes,activated partial thromboplastin time and prothrombin time were lower in two groups than before treatment,and those in electrotomy group were lower than those in curettage group,and the differences were statistically significant(all P<0.05).After treatment,the amount of menstruation,the thickness of endometrium under ultrasound,the degree of menstrual pain and the duration of menstruation in the two groups were lower than before treatment,and the electrotomy group was lower than the curettage group,serum hemoglobin concentration and menstrual cycle regularity in two groups were higher than those before treatment,and electrotomy group were higher than curettage group,and the differences were statistically significant(all P<0.05).The total effective rate of electrotomy group was 98.00%,higher than that of curettage group 82.00%,and the complication rate(2.00%)was lower than that of curettage group(16.00%),and the differences were statistically significant(χ^(2)=5.005,4.396,both P<0.05).Conclusion In patients with abnormal uterine bleeding,surgical treatment can obtain better curative effect,while hysteroscopic electrotomy can si
