外用加味过敏煎联合西替利嗪治疗丘疹性荨麻疹的临床观察

摘　　要：目的:观察外用加味过敏煎联合西替利嗪治疗丘疹性荨麻疹的临床效果及安全性。方法:将2022年1月—2023年6月于毕节市中医医院就诊的200例丘疹性荨麻疹患者随机分为治疗组和对照组,各100例。治疗组予盐酸西替利嗪口服溶液口服联合外用加味过敏煎治疗,对照组予盐酸西替利嗪口服溶液口服联合外用地奈德乳膏治疗。对患者瘙痒程度、丘疹数目及瘙痒发作频率进行评分,并记录不良反应和复发情况,判定临床疗效。结果:治疗前、治疗10 d后、治疗后20 d,治疗组与对照组瘙痒程度和丘疹数目、瘙痒发作频率评分比较,差异均无统计学意义(P>0.05);治疗后20 d,治疗组与对照组的瘙痒程度和丘疹数目、瘙痒发作频率评分均低于治疗10 d后,治疗10 d后均低于治疗前,差异均有统计学意义(P<0.05)。治疗10 d后,治疗组总有效率为74.5%,对照组为60.0%,两组比较,差异有统计学意义(χ^(2)=4.380,P=0.036);治疗后20 d,治疗组总有效率为89.4%,对照组为85.6%,两组比较,差异无统计学意义(χ^(2)=0.609,P=0.435)。治疗组复发率为23.68%,对照组为28.57%,两组比较,差异无统计学意义(χ^(2)=0.452,P>0.05)。两组均未见不良反应发生。结论:短期外用加味过敏煎联合西替利嗪治疗丘疹性荨麻疹的效果优于地奈德乳膏联合西替利嗪的效果,外用加味过敏煎能有效改善丘疹性荨麻疹患者皮肤瘙痒及皮损等情况。Objective:To observe the clinical effect and safety of Jiawei Guomin Decoction of external application combined Cetirizine in the treatment of papular urticaria.Method:A total of 200 patients with papular urticaria treated in Bijie Traditional Chinese Medicine Hospital from January 2022 to June 2023 were randomly divided into treatment group and control group,with 100 cases in each group.The treatment group was treated with Cetirizine Hydrochloride Oral Solution combined with Jiawei Guomin Decoction of external application,and the control group was treated with Cetirizine Hydrochloride Oral Solution combined with Desonide Cream of external application.The pruritus degree,papules number and pruritus attack frequency of patients were scored,the adverse reactions and recurrence were recorded,and the clinical efficacy were determined.Result:There were no significant differences between the treatment group and the control group before treatment,after 10 days of treatment and 20 days after treatment in the scores of pruritus degree,papules number and pruritus attack frequency(P>0.05).20 days after treatment,the scores of pruritus degree,papules number and pruritus attack frequency in the treatment group and the control group were lower than those after 10 days of treatment,and those after 10 days of treatment were lower than those before treatment,the differences were statistically significant(P<0.05).After 10 days of treatment,the total effective rate was 74.5%in the treatment group and 60.0%in the control group,the difference was statistically significant(χ^(2)=4.380,P=0.036).20 days after treatment,the total effective rate was 89.4%in the treatment group and 85.6%in the control group,there was no significant difference between the two groups(χ^(2)=0.609,P=0.435).The recurrence rate was 23.68%in the treatment group and 28.57%in the control group,there was no significant difference between the two groups(χ^(2)=0.452,P>0.05).No adverse reactions occurred in both groups.Conclusion:The effect of short-term topical Jia
