摘 要:目的 了解创伤分泌物标本中病原菌分布特征及主要病原菌的耐药性,为创伤感染的治疗提供依据。方法 收集3202例创伤分泌物标本微生物培养结果,回顾性分析分离菌种的构成及分离率较高菌种的耐药性。结果 培养阳性率53.19%,共收集菌株2172株。分离率排在前5位的菌种是大肠埃希菌(19.48%)、金黄色葡萄球菌(18.69%)、凝固酶阴性葡萄球菌(13.21%)、肺炎克雷伯菌肺炎亚种(4.97%)及阴沟肠杆菌复合群(4.05%)。ESBLs检出率57.68%,MRSA检出率25.37%。结论 创伤分泌物标本微生物培养阳性率较高,菌种多样化,各菌种的耐药率差异也较大。临床医生需重视创伤感染标本的微生物培养监测,慎重使用抗菌药物,在发生创伤感染时清创治疗比使用抗菌药物更为重要。Objective To understand the distribution characteristics of pathogenic bacteria in wound secretion specimens and the drug resistance of the main pathogens,and to provide the basis for the treatment of wound infections.Methods The microbial culture results of 3202 cases of wound secretion specimens were collected,and the composition of the isolated strains and the drug resistance of the strains with higher isolation rate were retrospectively analyzed.Results The positive culture rate of the samples was 53.19%,and 2172 bacterial strains were collected.The top 5 isolated bacteria were Escherichia coli(19.48%),Staphylococcus aureus(18.69%),coagulase negative staphylococcus(13.21%),Klebsiella pneumoniae subsp(4.97%)and Enterobacter cloacae complex group(4.05%).The detection rate of ESBLs was 57.68%,and that of MRSA was 25.37%.Conclusions The positive rate of microorganism culture in the specimen of wound secretions is higher,the strains are diversified,and the drug resistance rate in different bacteria is also different.Clinicians should pay more attention to the monitoring of microbial of trauma infection specimens and use antibiotics carefully.Debridement treatment is more important than the use of antibiotics in the event of trauma infection.
