从“肾虚精亏,精室失用”探讨良性前列腺增生的辨治

摘　　要：中医学认为肾气亏虚、肾精不足是良性前列腺增生发病之本,主要病位在肾与精室,临床多表现为排尿困难、淋漓不尽等本虚标实之症。肾主导精室,前列腺属精室范畴,本文通过梳理肾与精室的关系,从“肾虚精亏,精室失用”理论出发,拟定补肾生精为核心治法,佐以活血化瘀、利湿祛浊、通阳散寒行气利水之法祛除精室邪实。同时注意多脏腑协调兼顾,通补结合,以清补为主,改善中老年男性下尿路症状,以供临床参考。Traditional Chinese medicine(TCM)believes that deficiency of kidney qi and essence is the root cause of benign prostatic hyperplasia(BPH),and the main problem is in the kidney and semen chamber.Clinically,the patient often presents with symptoms such as difficulty urinating and incomplete dribbling,which are out of deficiency in root and excess in superficiality.The kidney dominates the semen chamber,and the prostate belongs to the category of semen chamber.By sorting out the relationship between the kidney and semen chamber,starting from the theory of“If kidney and essence are in deficiency,semen chamber will be in dysfunction”,the core treatment of supplementing the kidney and generating essence can be formulated,which can be assisted with the methods of activating blood and resolving stasis,eliminating dampness and turbidity,moving qi and assisting diuresis,unblocking yang and dispersing cold to eliminate the pathogen in the semen chamber.At the same time,we should also focus on the coordination of multiple viscera,the combination of unblocking and tonifying,to improve the middle aged and elderly males lower urinary tract symptoms.
