填精育嗣方治疗肾精亏损型少弱精子症的临床研究

摘　　要：目的:观察填精育嗣方对肾精亏损型少弱精子症患者的有效性和安全性。方法:纳入中国中医科学院西苑医院肾精亏损型少弱精子症患者69例,采用随机数字表法随机分为试验组和对照组,试验组应用填精育嗣方治疗,对照组应用左卡尼汀口服溶液治疗,疗程均为12周;试验期间脱落及剔除5例,最终可供评估有效病例64例,试验组32例,对照组32例;观察两组治疗前后精液参数、中医证候评分及性激素水平。结果:与治疗前相比,两组精子密度、PR、PR+NP均提高,DFI降低,差异具有统计学意义(P<0.05),组间比较,两组精子密度、PR、PR+NP提高、DFI降低,但差异无统计学意义(P>0.05)。与治疗前相比,对照组中医证候评分、睾酮水平无显著提升(P>0.05);试验组中医证候评分、睾酮水平提高,差异有统计学意义(P<0.05),与对照组比较差异具有统计学意义(P<0.05)。两组治疗中未见严重不良反应,治疗后安全性指标未见异常。结论:填精育嗣方可提升肾精亏损型少弱精子症患者精子密度、活动率及睾酮水平,降低DFI,并改善中医证候评分,且安全性良好。Objective:To observe the efficacy and safety of the Tianjing Yusi formula(TYF)in the treatment of patients with oligoasthenospermia of kidney essence deficiency type.Method:Sixty-nine patients with oligoasthenospermia of kidney essence deficiency type in Xiyuan Hospital,China Academy of Chinese Medicine Sciences were included and randomly divided into test group and control group.The test group received TYF and the control group received levocarnitine oral solution.Both groups were administered 12 weeks.During the trial,5 cases dropped,and 64 valid cases were finally available for evaluation,including 32 cases in the test group and 32 cases in the control group.Semen parameters,TCM syndrome score and sex hormone levels were observed before and after treatment.Results:Compared with pre-treatment,sperm density,PR,and PR+NP improved,DFI decreased in both groups with statistical differences(P<0.05),and there was no statistical difference between the two groups(P>0.05).Compared with the pre-treatment record,TCM syndrome score and testosterone level in control group were not significantly improved(P>0.05).TCM syndrome score and testosterone level of experimental group increased,and the differences were statistically significant(P<0.05),compared with control group(P<0.05).No serious adverse reactions were observed in the two groups,and no abnormal safety indexes were observed after treatment.Conclusion:Tianjing Yusi formula can improve the sperm density,motility rate and testosterone level,reduce DFI,and improve the TCM syndrome score in patients with oligonasthenospermia of kidney essence deficiency type,and has good safety.
