复方羌活通络合剂联合悬灸治疗风邪上扰型后循环缺血性眩晕的临床研究

摘　　要：目的:探讨复方羌活通络合剂联合悬灸治疗风邪上扰型后循环缺血性眩晕(PCIV)的临床疗效。方法:选择2021年9月—2023年9月于海安市中医院就诊的风邪上扰型PCIV患者126例,采用随机数字表法分成悬灸组与联合组,每组63例。悬灸组采用悬灸治疗,联合组采用复方羌活通络合剂联合悬灸治疗。对两组患者治疗前后的中医证候积分、眩晕程度、血液指标、血流动力学指标、临床疗效与不良反应发生情况进行对比。结果:治疗后,联合组中医证候积分低于悬灸组(P<0.01);联合组左、右椎动脉及基底动脉平均血流速度均高于悬灸组(P<0.01);联合组血浆D-2聚体、脂蛋白磷脂酶A2(LP-PLA2)、纤维蛋白原(FIB)均低于悬灸组(P<0.01);联合组眩晕障碍量表(DHI)评分低于悬灸组(P<0.01);联合组患者治疗总有效率高于悬灸组(P<0.01);两组不良反应总发生率差异无统计学意义(P>0.05)。结论:复方羌活通络合剂联合悬灸能够改善患者血液指标及血流动力学指标,降低中医证候积分及眩晕程度,提高患者临床疗效,安全性较高。Objective:To investigate the clinical effects of compound Qianghuo Tongluo mixture(CQTM)combined with suspended moxibustion in the treatment of post-circulation ischemic vertigo(PCIV)of wind pathogen harassing the upper body type.Methods:A hundred and twenty-six patients with wind pathogen harassing the upper body type PCIV who were treated at Hai an Hospital of Traditional Chinese Medicine from September 2021 to September 2023 were selected and randomly divided into a suspended moxibustion group and a combination group using a random number table method,with 63 cases in each group.Traditional Chinese medicine(TCM)syndrome score,degree of vertigo,blood index,hemodynamic index,clinical efficacy and adverse reactions were compared between the two groups before and after treatment.Results:After treatment,the TCM syndrome score of combination group was lower than that of suspended moxibustion group(P<0.05);The average blood flow velocity of left and right vertebral artery and basilar artery in combination group was higher than that in suspended moxibustion group(P<0.05).Plasma D-2 polymers,lipoprotein phospholipase A2(LP-PLA2)and fibrinogen(FIB)in combination group were lower than those in suspended moxibustion group(P<0.05).The score of vertigo disorder scale(DHI)in combination group was lower than that in suspended group(P<0.05).The total effective rate of combination group was higher than that of suspended moxibustion group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compound Qianghuo Tongluo mixture combined with suspended moxibustion can improve blood index and hemodynamic index of patients,reduce TCM syndrome score and vertigo degree,and improve clinical efficacy and high safety.
