蜈蚣败毒饮联合肤舒止痒膏治疗寻常型银屑病血热风燥型的临床疗效及对免疫因子CXCL9、CXCL10的影响

摘　　要：目的:观察蜈蚣败毒饮联合肤舒止痒膏治疗寻常型银屑病血热风燥型的临床疗效,以及对免疫相关因子CXCL9、CXCL10的影响。方法:选取黑龙江中医药大学附属第一医院皮肤科2021年5月—2023年5月就诊的寻常型银屑病血热风燥型患者72例,随机分为治疗组与对照组,每组36例。试验中脱落6例,最终治疗组35例,给予口服蜈蚣败毒饮联合外用肤舒止痒膏治疗,对照组31例,仅口服消银胶囊治疗。记录治疗前、治疗后第2周和第4周的皮损面积和严重程度指数(PASI)、皮肤病生活质量指数(DLQI)、中医证候评分进行差异性比较,并检测外周血中CXCL9和CXCL10含量。结果:两组治疗后第2、4周PASI评分、DLQI评分和中医证候评分均明显低于治疗前,且治疗组患者明显低于对照组,差异均有统计学意义(P<0.05)。两组血清中CXCL9和CXCL10含量大幅度下降(P<0.05),且治疗组趋化因子含量远低于对照组(P<0.05)。结论:蜈蚣败毒饮联合肤舒止痒膏能有效改善皮肤病变状况及症状,提升患者生活质量,且安全性高,适合临床推广使用。Objective:To observe the clinical efficacy of Wugong Baidu Yin(WBY)combined with Fushu Zhiyang ointment(FZO)in the treatment of psoriasis vulgaris of blood heat and wind dryness type,and its impact on immune factors CXCL9 and CXCL10.Methods:Seventy-two patients with psoriasis vulgaris of the blood-heat and wind-dry type who visited the Dermatology Department of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from May 2021 to May 2023 were randomly divided into treatment group and control group,with 36 patients in each group.In the trial,6 cases dropped out,35 patients in the treatment group were treated with WBY combined with FZO;while 31 cases in the control group were treated with Xiaoyin capsule.Psoriasis area and severity index(PASI),dermatology life quality index(DLQI),and TCM syndrome scores were recorded before treatment for differential comparisons,and the levels of CXCL9 and CXCL10 in peripheral blood were detected.Results:PASI scores,DLQI scores and TCM symptom scores were significantly lower than those before treatment in both groups at weeks 2 and 4 after treatment,and they were significantly lower in the treatment group than those in the control group,and the differences were all statistically significant(P<0.05).The levels of CXCL9 and CXCL10 in the serum of both groups decreased substantially(P<0.05),and the level of chemokines was much lower in the treatment group than in the control group(P<0.05).Conclusion:Wugong Baidu yin combined with Fushu Zhiyang ointment can effectively treat psoriasis vulgaris of blood heat and wind dryness type,improve the quality of life of patients,and is safe and suitable for clinical promotion.
