生化汤及其加减方治疗药物流产后阴道出血的研究进展

摘　　要：生化汤是产后病血瘀证的基础方。现代临床中,除应用生化汤原方外,其加减方也用于药物流产后阴道出血的治疗,并可与其他方剂(如失笑散、王不留行散)、中成药(如益母草颗粒、宫血宁胶囊)、西药(如雌孕激素、缩宫素、米非司酮)、中医针刺或耳穴压豆等特色疗法联用。临床疗效显示生化汤及其加减方能缩短阴道出血时间,减少出血量,促进月经复潮,改善性激素水平,降低清宫率且不良反应少。实验药理学研究显示,生化汤及其加减方通过促进细胞凋亡、调节血管生成、改善微循环、调节炎症因子、调控免疫细胞功能以及调节脂肪酸与氨基酸代谢,发挥防治药物流产后阴道出血的作用。本文就生化汤及其加减方治疗药物流产后阴道出血的临床和实验研究进行系统综述,以期为中医药临床治疗药物流产后阴道出血提供一定的思路。Shenghua decoction is the basic formula for postpartum blood stasis syndrome.In modern clinical application,in addition to the original formula of Shenghua Decoction,its modified formulae are also used for the treatment of vaginal bleeding after medical abortion(VBMA).The formula can also be used in combination with other formula(Shixiao powder,cowherb seed powder),Chinese patent medicine(motherwort granule,Gongxuening capsule),Western medicine(estrogen,progestational hormone,oxytocin,mifepristone),traditional Chinese acupuncture or ear point pressure bean and other characteristic therapies.The clinical efficacy results showed that Shenghua decoction and its modified formulae can shorten the vaginal bleeding time and reduce the amount of bleeding,promote menstrual relapse,improve the level of sex hormones,reduce the rate of uterine evacuation and have few adverse reactions.Experimental pharmacological studies have shown that the modified Shenghua decoction promotes cell apoptosis,regulates angiogenesis,facilitates endometrial repair,improves microcirculation and coagulation function,promotes uterine contraction,regulates inflammatory factors,modulates immune cell function,and regulates fatty acid and amino acid metabolism.It plays a role in preventing and treating VBMA.This article provides a systematic review of the clinical and basic research progress on the use of Shenghua decoction in the treatment of VBMA.
