摘　　要：目的 将中文版简版言语空间听觉质量量表(C-SSQ12)在临床上用于老年性聋的听力评估,并将C-SSQ12得分与纯音听阈值进行比较,分析C-SSQ12得分与老年性听力损失的相关性。方法 选取2022年10月—2023年10月首都医科大学附属北京朝阳医院耳鼻咽喉头颈外科门诊患者411例,其中老年性聋患者296例,根据纯音听阈测试言语频率均值(4f-PTA),分为轻度听力损失组(20≤4f-PTA<35 dB HL)141例、中度听力损失组(35≤4f-PTA<50 dB HL)78例。中重度听力损失组(50≤4f-PTA<65 dB HL)59例和重度及以上听力损失组(4f-PTA≥65dB HL)18例,正常听力组(4f-PTA<20 dB HL)115例,完成C-SSQ12填写和纯音听阈测试。采用Pearson相关性分析,观察老年性聋患者4f-PTA与C-SSQ12得分及其在言语感知维度、空间听觉维度以及听觉质量维度得分的相关性。利用单因素方差分析比较轻度、中度、中重度、重度及以上听力损失组C-SSQ12得分差异。绘制ROC曲线,计算曲线下面积(AUC)评估C-SSQ12对显著性听力损失的诊断效能。结果 C-SSQ12得分及其在言语感知维度、空间听觉维度以及听觉质量维度得分均与老年性聋患者4f-PTA呈显著负相关(P <0.01)。针对C-SSQ12绘制ROC曲线,AUC为0.884,敏感性为82.9%、特异性为79.2%、阳性预测值为62.5%、阴性预测值为93.1%,约登指数为0.62,截断值为6.56分。结论 C-SSQ12得分与纯音听阈值呈显著负相关,C-SSQ12具有良好的敏感性、特异性与诊断效能,可作为听力检查的有效补充,得分能够反映老年性聋患者在言语感知、空间听觉以及听觉质量维度下的听功能障碍,可用于老年性聋的筛查。Objective To determine the correlation between C-SSQ12 scores and 4-frequency pure tone average(4f-PTA)for their potentials in screening for presbycusis.Methods Patients recruited from at the department of otolaryngology,head and neck surgery of Beijing Chaoyang Hospital from October 2022 to October 2023(n=411)were selected,including 296 elderly patients with hearing loss.Based on their 4f PTA,they were categorized as having mild hearing loss(20-35 dB HL,n=141),moderate hearing loss(35-50 dB HL,n=78),moderate to severe hearing loss(50-65 dB HL,n=59),or severe and worse hearing loss(≥65 dB HL,n=18).Participants with normal hearing(<20 dB HL,n=115)also completed the C-SSQ12 and pure tone audiometry.The correlation between 4f-PTA and C-SSQ12 scores and the scores of its speech,spatial and qualities of hearing dimensions in patients with hearing loss were analyzed using Pearson correlation analysis.C-SSQ12 scores were compared among patients with different degrees of hearing loss using one-way ANOVA,and post hoc multiple comparison using Bonferroni correction.ROC and the area under the curve(AUC)were used to evaluate the diagnostic efficacy of C-SSQ12 in patients with significant hearing loss.Results The total C-SSQ12 score and scores in the speech,space and qualities dimensions were negatively correlated with 4f-PTA(P<0.001).Prediction using ROC with an AUC of 0.884 showed a sensitivity of 82.9%,a specificity of 79.2%,a positive predictive value of 62.5%,and a negative predictive value of 93.1%,with the Yoden index at 0.62,and a cutoff value at 6.56.Conclusions C-SSQ12 scores are negatively correlated with pure tone hearing thresholds,with satisfying sensitivity,specificity and diagnostic efficacy.They can be used as an effective supplement to hearing tests.The scores reflect hearing impairment in elderly patients in speech,spatial and qualities of hearing,and can be used for screening for presbycusis.
