摘  要:目的:探讨基于“康复团队-街道工作者-患者-家属”四位一体的微信平台管理在心力衰竭患者康复中的应用。方法:选取2021年1月-2022年12月在某院就诊的84例心力衰竭患者的临床资料进行研究。按照基于“康复团队-街道工作者-患者-家属”四位一体的微信平台管理实施时间前后,将2021年1-12月使用常规出院后管理模式的40例患者设为常规组,将2022年1-12月使用基于“康复团队-街道工作者-患者-家属”四位一体的微信平台管理模式的44例患者设为微信组。比较两组患者的心功能康复情况[左心室收缩末期体积(LVESV)、左心室舒张末期体积(LVEDV)、左心室射血分数(LVEF)]、心理状态[7项广泛性焦虑量表(GAD-7)、简单自测抑郁量表(PHQ-9)]、疾病认知水平(中文版心力衰竭知识问卷)、自我管理行为[心力衰竭自我护理指数量表(SCHFI)]、生活质量[明尼苏达心力衰竭生活质量量表(MLHFQ)]、再入院率以及护理满意度。结果:干预前,两组患者LVESV、LVEDV、LVEF、GAD-7评分、PHQ-9评分、中文版心力衰竭知识问卷评分、SCHFI各维度评分、MLHFQ各维度评分比较均无统计学差异(P>0.05)。干预后,两组患者LVEF、中文版心力衰竭知识问卷评分、SCHFI各维度评分均上升,且微信组患者高于常规组(P<0.05)。两组患者GAD-7评分、PHQ-9评分、LVESV、LVEDV、MLHFQ各维度评分均下降,且微信组患者低于常规组(P<0.05);微信组患者的再入院率9.09%低于常规组再入院率27.50%(P<0.05);微信组患者的护理满意度93.18%高于常规组的护理满意度77.50%(P<0.05)。结论:基于“康复团队-街道工作者-患者-家属”四位一体的微信平台管理在心力衰竭患者康复中应用良好,可改善患者心功能、心理状态,提高患者疾病认知水平、自我管理行为、生活质量、护理满意度,进而降低再入院率。Objective:To explore the application of Wechat platform management based on rehabilitation teamstreetworker-patient-family quaternity in the rehabilitation of patients with heart failure.Methods:The clinical data were collected from 84 patients with heart failure treated in the hospital between January 2021 and December 2022.According to the implementationof Wechat platform management based on rehabilitation team-street worker-patient-family quaternity,they were divided into routine group(40 cases,routine post-discharge management,before implementation,between January and December 2021)and Wechat group(44 cases,Wechat platform management based on rehabilitation team-street worker-patient-family quaternity,after implementation,between January and December 2022).The rehabilitation of cardiac function[left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV),left ventricular Ejection fraction(LVEF)],psychologicalstate[Generalized AnxietyDisorder-7(GAD-7),Patient Health Questionnaire-9(PHQ-9)],disease cognition(heart failure knowledge questionnaire of Chinese version),self-management behaviors[Self-Care of Heart Failure Index(SCHFI)],quality of life[Minnesota Living with Heart Failure Questionnaire(MLHFQ)],readmission rate and nursing satisfaction were compared between the two groups.Results:Before intervention,there was no significant difference in LVESV,LVEDV,LVEF,scores of GAD-7,PHQ-9,heart failure knowledge questionnaire,SCHFI and MLHFQ between the two groups(P>0.05).After intervention,LVEF,scores ofheart failure knowledge questionnaire of Chinese version and SCHFI were increased in both groups,which were higher in wechat group than routine group(P<0.05),while scores of GAD-7 and PHQ-9,LVESV,LVEDV and MLHFQ score were decreased,which were lower in Wechat group than routine group(P<0.05).The readmission rate in Wechat group was lower than that in routine group(9.09%vs 27.50%,P<0.05),and nursing satisfaction was higher than that in routine group(93.18%vs77.50%,P<0.05).Conclusion:Appli
