藤花脉络复新方内服与黄连膏外敷联合西医治疗下肢血栓性浅静脉炎继发静脉溃疡的效果观察

摘　　要：目的 观察藤花脉络复新方内服与黄连膏外敷联合西医对下肢血栓性浅静脉炎继发静脉溃疡的治疗效果。方法 前瞻性选取2019年1月—2020年12月接受治疗的115例下肢血栓性浅静脉炎继发静脉溃疡患者为研究对象,并根据随机数字表法将其分为对照组(50例)和观察组(65例),两组均给予常规西药治疗,观察组在此基础上给予藤花脉络复新方内服与黄连膏外敷。比较两组的治疗效果、溃疡面积和疼痛评分、炎症因子水平和生活质量的差异。结果 观察组患者的治疗有效率为100.00%,明显高于对照组的88.00%,差异有统计学意义(χ^(2)值=5.982,P=0.014);两组治疗前疼痛评分和溃疡面积无差别,治疗2周后,观察组患者的疼痛评分低于对照组,溃疡面积较对照组小,差异有统计学意义(P<0.05);两组治疗前炎症因子水平无差别,治疗2周后,观察组的CRP、IL-6和TNF-α水平低于对照组,差异有统计学意义(P<0.01);两组治疗前生活质量无差别,治疗2周后,两组生活质量均较治疗前改善,且观察组优于对照组,差异有统计学意义(P<0.05)。结论 藤花脉络复新方内服与黄连膏外敷联合西医对下肢血栓性浅静脉炎继发静脉溃疡有良好的治疗效果,可改善炎症因子水平和生活质量,有良好的应用价值。Objective This study aims to observe the therapeutic effects of the combined treatment of internal administration of Tenghua Mailuo Fuxin Decoction(藤花脉络复新方)and external application of Huanglian Ointment(黄连膏)with conventional Western medicine on secondary venous ulcers due to thrombophlebitis of the lower extremities.Methods A prospective cohort of 115 patients with secondary venous ulcers due to lower extremity thrombophlebitis treated at our hospital from January 2019 to December 2020 was selected.These patients were randomly assigned to either the control group(50 patients)or the observation group(65 patients).Both groups received standard Western medical treatment,while the observation group also received Tenghua Mailuo Fuxin Decoction internally and Huanglian Ointment externally.The treatment outcomes,ulcer area reduction,pain scores,levels of inflammatory markers,and quality of life improvements were compared between the two groups.Results The effective treatment rate in the observation group was 100.00%,significantly higher than the 88.00%in the control group(χ^(2)=5.982,P=0.014).Both groups showed no differences in pain scores and ulcer sizes before treatment.After two weeks,the observation group reported lower pain scores and smaller ulcer sizes than the control group(P<0.05).Inflammatory markers,including CRP,IL-6,and TNF-αlevels,were similar before treatment but were significantly lower in the observation group after two weeks(P<0.01).Both groups exhibited improved quality of life after treatment compared to before,with the observation group showing greater improvements(P<0.05).Conclusion The combined treatment of Tenghua Mailuo Fuxin Decoction and Huanglian Ointment with conventional Western medicine is highly effective for treating secondary venous ulcers due to lower extremity thrombophlebitis,improving inflammatory markers and quality of life,thus presenting significant clinical value.
