基于脾肾同调理论探讨平胃散合用五苓散治疗慢性痛风性关节炎的机制

摘　　要：痛风是一种嘌呤代谢紊乱,尿酸合成过多或尿酸分解及排泄障碍引起尿酸在血液中积聚,使血液或组织液中的尿酸超过其饱和度形成单钠尿酸盐,沉积于关节及其他组织内,引起关节疼痛、畸形、痛风石、功能障碍等病理性改变的疾病,是生理与病理综合作用的结果。文章从脾、肾的生理功能出发,探究脾、肾与痛风发生发展的关联,认为水精不布、湿浊蓄积是痛风发生发展的关键,脾肾亏虚、骨髓失充是痛风迁延难愈的根本。针对脾肾的生理特点及该病的发病特点,采用平胃散合用五苓散,通过健脾燥湿化浊以调补脾胃(土)的功能,温肾化气利水以恢复肾脏(水)的功能,达到脾(胃)肾同治的目的。文章将根据脾肾同调理论对平胃散合并五苓散治疗慢性痛风性关节炎的机制进行探讨,以期为慢性痛风性关节炎的研究及临床治疗提供思路。As the result of the comprehensive interaction of physiology and pathology,gout is a disorder resulting from monosodium urate deposited in the joints and other tissues formed by uric acid accumulated above its saturation in the blood or tissue fluid due to disordered purine metabolism as well as excessive synthesis or obstructive decomposition and excretion of uric acid,causing joint pain,deformity,tophus,dysfunction and other pathological changes.Based on the physiological functions of spleen and kidney,this paper explores the relationship of spleen and kidney,and the occurrence and development of gout,considering the uneven distribution of body fluid and nutrition as well as the accumulation of moisture and turbidity is the key to the occurrence and development of gout,and the deficiency of spleen and kidney as well as the degeneration of bone marrow is the root of gout difficult to treat through a long time.In view of the physiological characteristics of spleen and kidney as well as the pathogenesis of this disease,Pingwei Powder(平胃散)and Wuling Powder(五苓散)were used to invigorate the spleen,remove dampness and dissolve turbidity to complement the function of spleen and stomach(soil),and to warm kidney,exhaust Qi and drain water to restore the function of kidney(water),so that spleen(stomach)and kidney are simultaneously treated.This article will discuss the mechanism of Pingwei Powder and Wuling Powder in the treatment of chronic gouty arthritis according to the theory of spleen-kidney synchronization,in order to provide ideas for the research and clinical treatment of chronic gouty arthritis.
