中西医结合论治咳嗽变异性哮喘

摘　　要：慢性咳嗽是呼吸门诊最常见的临床症状,其中三分之一的慢性患者均为咳嗽变异性哮喘,因其发病的隐匿性,临床上经常被忽视或误诊,对患者自身的工作、生活以及社会活动都造成了严重的影响。文章从西医病理机制,中医病因病机,及中西医治疗等方面论述咳嗽变异性哮喘的临床治疗经验。咳嗽变异性哮喘的发作主要与气道炎症、气道高反应性及气道重塑密切相关,治疗主要通过吸入糖皮质激素及长效β2受体激动剂为主。但多数患者无法接受长期的激素类药物治疗,因此,中医药的治疗则突显其优势。通过长期的临床实践,总结出咳嗽变异性哮喘的发病是以肺虚为本,风邪外扰,引动肺络伏饮,气道挛急,肺失宣降所致。中医从风、饮、虚的角度进行论治,联合西药临床疗效显著。也为进一步研究咳嗽变异性哮喘提供理论依据。Chronic cough is the most common clinical symptom in respiratory outpatient department.One third of chronic cough patients are cough variant asthma.Because of its hidden onset,it is often ignored or misdiagnosed clinically,which has a serious impact on patients work,life and social activities.This paper discusses the clinical treatment experience of cough variant asthma from the aspects of Western medicine pathological mechanism,traditional Chinese medicine etiology,and Traditional Chinese and Western medicine treatment.The onset of cough-variant asthma is mainly closely related to airway inflammation,airway hyperresponsiveness and airway remodeling,and the treatment is mainly achieved through inhaled glucocorticoids and long actingβ2 receptor agonists.But most patients are not under long-term hormonal therapy,so traditional Chinese medicine treatment highlights its advantages.Through the clinical practice,we conclude that cough variant asthma is caused by lung deficiency.Wind evil induced phlegm and retained fluid of lung,lead to tracheospasm,this is the process of cough-variant asthma.Cough variant asthma,traditional Chinese medicine from the perspective of wind,drinking,deficiency,combined with Western medicine clinical effect is remarkable.This article provides a rationale for further research on cough-variant asthma.
