肛痈促愈汤对肛周脓肿术后创面愈合的影响

摘　　要：目的 评价肛痈促愈汤用于肛周脓肿术后治疗的效果。方法 选取2020年2月—2023年2月合肥市第八人民医院肛肠科收治的肛周脓肿患者90例,分为对照组与观察组,各45例,方法为随机数字表法。所有患者均接受肛周脓肿根治术,对照组患者于术后进行高锰酸钾坐浴,观察组则在对照组治疗方案上加用肛痈促愈汤,两组患者均持续治疗3周。比较两组治疗后临床疗效,创面愈合情况,治疗前后炎症反应、血清表皮生长因子(EGF)、血管内皮生长因子(VEGF)及表皮生长因子受体(EGFR)、碱性成纤维细胞生长因(bFGF)表达水平以及治疗期间的安全性。结果 观察组临床总有效率高于对照组;观察组患者上皮出现时间、创面腐肉脱落时间均短于对照组,创面愈合率高于对照组;与治疗前比较,治疗3周后,两组血清SAA、CRP、ACTA、IL-6水平降低,且观察组更低;治疗3周后两组血清EGF、VEGF水平及EGFR、bFGF表达水平升高,且观察组更高,差异具有统计学意义(P<0.05);两组安全性对比,差异无统计学意义(P>0.05)。结论 肛周脓肿患者术后应用肛痈促愈汤效果较好,可加快患者创面愈合进程,减轻炎症反应,促进组织再生修复,且安全性良好。Objective To evaluate the effect of Gangyong Chuyu Decoction(肛痈促愈汤)in the postoperative treatment of perianal abscess.Methods A total of 90 patients with perianal abscess admitted to the department of proctology of the eighth people's hospital of Hefei city from February 2020 to February 2023 were selected and divided into the control group and the observation group,45 cases in each group,using random number table method.All patients received radical resection of perianal abscess,the control group received potassium permanganate sitbath after surgery,and the observation group was treated with Gangyong Chuyu Decoction in addition to the treatment plan of the control group.Both groups received continuous treatment for 3 weeks.The clinical efficacy,wound healing,inflammatory response,serum epidermal growth factor(EGF),vascular endothelial growth factor(VEGF),epidermal growth factor receptor(EGFR),basic fibroblast growth factor(bFGF)expression levels before and after treatment and safety during treatment were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group.The time of epithelium emergence and wound rotting shedding of the observation group was shorter than that of the control group,and the wound healing rate was higher than that of the control group.Compared with before treatment,after 3 weeks of treatment,the serum levels of SAA,CRP,ACTA and IL-6 of the two groups decreased,and the observation group was lower.After 3 weeks of treatment,serum EGF and VEGF levels and EGFR and bFGF expression levels of the two groups increased,and the observation group was higher,and the differences were statistically significant(P<0.05).There was no significant difference in safety between the two groups(P>0.05).Conclusion The treatment effect of Gangyong Chuyu Decoction is good in patients with perianal abscess,which can accelerate wound healing process,reduce inflammatory reaction,promote tissue regeneration and repair with good safety.
