摘 要:目的 探讨维生素D受体(VDR)基因BsmⅠ位点多态性与急性心肌梗死(AMI)的关系。方法 本研究为病例对照研究,选取2022—2023年西安市第九医院收治的AMI患者157例为病例组,另选取同期西安市第九医院收治的非AMI患者162例为对照组。收集患者一般资料、实验室检查指标及VDR基因BsmⅠ位点基因型及等位基因。采用多因素Logistic回归分析探讨AMI的影响因素。结果 病例组空腹血糖、总胆固醇、低密度脂蛋白胆固醇高于对照组,高密度脂蛋白胆固醇、血浆25-羟维生素D水平低于对照组(P<0.05)。两组VDR基因BsmⅠ位点基因型分布比较,差异有统计学意义(P<0.05);两组VDR基因BsmⅠ位点等位基因频率比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,总胆固醇〔OR=1.848,95%CI(1.369~2.495)〕、高密度脂蛋白胆固醇〔OR=0.040,95%CI(0.019~0.105)〕、血浆25-羟基维生素D水平〔OR=0.962,95%CI(0.935~0.990)〕、VDR基因BsmⅠ位点基因型〔TT与CC:OR=4.781,95%CI(1.703~13.424);CT与CC:OR=1.059,95%CI(0.600~1.870)〕为AMI的独立影响因素(P<0.05)。结论 VDR基因BsmⅠ位点基因型是AMI的独立影响因素,其中VDR基因BsmⅠ位点TT基因型可能是AMI的风险因素。Objective To explore the relationship between vitamin D receptor(VDR)gene BsmⅠlocus polymorphism and acute myocardial infarction(AMI).Methods This study was a case-control study,a total of 157 patients with AMI admitted to the Ninth Hospital of Xi'an from 2022 to 2023 were selected as the case group,and 162 non AMI patients admitted to the Ninth Hospital of Xi'an during the same period were selected as the control group.General information,laboratory test indicators,VDR gene BsmⅠlocus genotypes and alleles were collected.Multivariate Logistic regression analysis was used to explore the influencing factors of AMI.Results The fasting blood glucose,total cholesterol,low-density lipoprotein cholesterol in case group were higher than those in control group,and high-density lipoprotein cholesterol and plasma 25-hydroxyvitamin D level were lower than those in control group(P<0.05).There was significant differences in genotype distribution of VDR gene BsmⅠlocus between the two groups(P<0.05).There was no significant difference in allele frequency of VDR gene BsmⅠlocus between the two groups(P>0.05).Multivariate Logistic regression analysis showed that total cholesterol[OR=1.848,95%CI(1.369-2.495)],high-density lipoprotein cholesterol[OR=0.040,95%CI(0.019-0.105)],plasma 25-hydroxyvitamin D level[OR=0.962,95%CI(0.935-0.990)],VDR gene BsmⅠlocus genotype[TT vs CC:OR=4.781,95%CI(1.703-13.424);CT vs CC:OR=1.059,95%CI(0.600-1.870)]were independent influencing factors for AMI(P<0.05).Conclusion VDR gene BsmⅠlocus genotype is an independent influencing factor for AMI,and the TT genotype at the VDR gene BsmⅠlocus may be a risk factor for AMI.
