湖北省医院复用医疗器械现场预处理现状研究

摘　　要：目的调查湖北省医院复用器械现场预处理现状。方法采用便利抽样法,利用自制的复用器械现场预处理现状调查表对湖北省各级医院消毒供应中心工作人员进行调查,并分析进行去除明显污物预处理的影响因素。结果共回收有效问卷394份,有效回收率为98.50%。病房、门诊与手术室分类预处理执行率分别为83.50%和89.09%,去除明显污物预处理执行率分别为77.16%和86.29%。分别有44.67%的病房、门诊以及40.86%的手术室预处理后器械表面仍有肉眼可见的污物残留。50.25%的消毒供应中心工作人员曾遭受锐器伤。分别有3.04%与17.26%的工作人员所在医疗机构采用生理盐水、消毒剂擦拭或浸泡进行预处理。病房、门诊进行预处理的独立影响因素为有用于预处理的工具、发现未做预处理及时反馈,而手术室则为有用于预处理的地点、发现未做预处理及时反馈。结论湖北省医疗机构污染器械现场预处理总体执行率较高,但效果有待进一步提升,少数医疗机构污染器械现场预处理方式有待改进。病房、门诊和手术室器械现场预处理的影响因素各有侧重,可通过加强沟通、提供预处理工具、增设或改进预处理地点、“跨科潜移”等方式进行针对性改进。Objective To investigate the current situation of on-site pretreatment of reusable devices in the hospital of Hubei Province.Methods By used convenience sampling method and a self-designed questionnaire for the current situation of on-site pretreatment on reusable equipment,a survey was conducted on the staff of the disinfection supply center of hospitals at all levels in Hubei Province and the influencing factors of pre-treatment for removing obvious pollutants were analyzed.Results A total of 394 valid questionnaires were collected,the recovery rate of which was 98.50%.The execution rate of classified pretreatment in wards and outpatient department and operating room were 83.50%and 89.09%,and the pretreatment for obvious dirt removal were 77.16%and 86.29%.In 44.67%of the wards and outpatient and 40.86%of the operating rooms,there were still visible dirt residues on the surface of the instruments after pretreatment.There were 50.25%of CSSD staff suffered sharp instrument injuries due to inadequate classified pretreatment in medical institutions.There were 3.04%and 17.26%of the staff work in medical institutions used normal saline and disinfectant wiping or soaking as the main methods of device pretreatment.The independent influencing factors of pretreatment in the wards and outpatient department had tools for pretreatment,and timely feedback without pretreatment,and the independent influencing factors of pretreatment in the operating room had sites for pretreatment and timely feedback without pretreatment.Conclusion The overall implementation rate of the on-site pretreatment of contaminated devices in medical institutions in Hubei Province is relatively high,but the effect remains to be further improved,the on-site pretreatment of contaminated instruments in a few medical institutions needs to be improved.The influencing factors of the on-site pretreatment of contaminated devices in wards and outpatient department and operating room have different emphasis,targeted improvements can be made by strengthening commu
