摘 要:目的:评估1.5T磁共振成像(MRI)应用在膝骨关节炎(KOA)诊断中的价值。方法:纳入2019年6月~2021年5月于本院就诊的80例疑似KOA患者为观察对象,均行X射线、1.5T MRI检查,参照关节镜结果,评价二者诊断效能(准确性、特异性、灵敏性)、病变检出结果。结果:①80例疑似KOA患者经关节镜确诊66例,X射线的诊断准确性(85.00%)、特异性(78.57%)、灵敏性(86.36%)与1.5TMRI(92.50%)、(85.71%)、(93.94%)无显著性差异,P>0.05;②1.5TMRI在韧带改变、半月板改变、滑膜增厚、腔内游离体、关节积液、关节软骨改变方面的检出率更高,X射线在骨质改变方面的检出率更高,P<0.05。结论:1.5TMRI能够有效诊断KOA,并且1.5TMRI的非骨性病变检出率较高。Objective:To evaluate the value of 1.5T magnetic resonance imaging in the diagnosis of degenerative osteoarthropathy of the knee(KOA).Methods:A total of 80 patients suspected of KOA from June 2019 to May 2021 were included.X-ray and 1.5T magnetic resonance examination were performed,and the diagnostic efficacy(accuracy,specificity,sensitivity)and lesion detection results of both were evaluated with reference to the results of arthroscopy.Results:①Among the 80 suspected KOA patients,66 cases were confirmed by arthroscopy.The diagnostic accuracy(85.00%),specificity(78.57%)and sensitivity(86.36%)of X-ray were no different from those of 1.5T magnetic resonance(92.50%),(85.71%)and(93.94%),P>0.05;②1.5T magnetic resonance had a higher detection rate of ligament change,meniscus change,synovial thickening,intracavitary free body,joint effusion and articular cartilage change,while X-ray had a higher detection rate of bone change(P<0.05).Conclusion:1.5T MRI can effectively diagnose KOA,and the detection rate of non-bone lesions of 1.5T MRI is high.
