双能量CT单能量图像减影法对肺动脉栓塞诊断的应用价值

摘　　要：目的:使用双能量CT单能量图像与虚拟平扫相减影的方法,观察其在肺动脉栓塞的应用价值。方法:回顾性分析2022年4月~12月在本院接受双能量CT的肺动脉CT血管成像检查的高度怀疑肺动脉栓塞患者62例。对比70keV单能量图减影虚拟平扫得到的减影图与70keV单能量图对肺动脉栓塞的诊断价值。比较1~3级肺动脉栓子及3级以下肺动脉栓子的检出率。结果:62例高度怀疑肺动脉栓塞的患者确诊的为56例,其中,减影图检出的有55例,单能量图检出43例;共发现75个栓子,减影图73个,单能量图61个。减影法对栓塞的敏感度与特异度及栓子检出率均高于单能量图(P<0.05)。减影图的栓子数符合率及栓子等级符合率高于单能量图(P<0.05)。结论:在临床高度怀疑肺动脉栓塞时可行能谱CT,单能量图减影虚拟平扫图比单能量图对栓子的检出率更高,可结合碘基图综合判断。Objective:To observe the application value of single-energy image and virtual plain scan subtraction in pulmonary embolism.Methods:A retrospective analysis was conducted on 62 highly suspected pulmonary embolism patients who underwent dual-energy CT angiography of the pulmonary artery in our hospital from April to December 2022.To compare the diagnostic value of 70keV single-energy subtraction virtual plain scan and 70keV single-energy imaging in pulmonary embolism.The detection rates of grade 1-3 pulmonary emboli and less than grade 3 pulmonary emboli were compared.Results:56 of 62 patients with highly suspected pulmonary embolism were diagnosed.Among the 56 patients with pulmonary embolism,55 cases were detected by subtraction map and 43 cases by single-energy map.A total of 75 emboli,73 subtraction maps and 61 single-energy maps were found.The sensitivity,specificity,and detection rate of emboli using subtraction method were higher than those using single-energy imaging(P<0.05).The consistency rate of the number and grade of emboli in the subtraction plot was higher than that in the single-energy plot(P<0.05).Conclusion:When pulmonary embolism is highly suspected in clinical practice,energy spectrum CT is feasible.Single-energy subtraction virtual plain scan has a higher detection rate for emboli than single-energy imaging,and can be combined with iodine based imaging for comprehensive judgment.
